Function and Efficacy
Penicillin has good antibacterial effect on Streptococcus such as hemolytic Streptococcus, Streptococcus pneumoniae and Staphylococcus that does not produce penicillinase. It has moderate antibacterial effect on enterococci, and Neisseria gonorrhoeae, Neisseria meningitidis, Corynebacterium diphtheriae, Bacillus anthracis, Actinomyces bovis, Streptobacillus candida, Listeria monocytogenes, Leptospira and Treponema pallidum are sensitive to this product. This product also has certain antibacterial activity against Haemophilus influenzae and Bordetella pertussis, and other Gram-negative aerobic or facultative anaerobic bacteria are less sensitive to this product. This product has good antibacterial effect on Clostridium, Peptostreptococcus anaerobic bacteria and melanin-producing Bacteroides, but has poor antibacterial effect on Bacteroides fragilis. Penicillin exerts its bactericidal effect by inhibiting bacterial cell wall synthesis.

Indication
Penicillin is suitable for various infections caused by sensitive bacteria, such as abscesses, bacteremia, pneumonia and endocarditis. Among them, penicillin is the first choice for the following infections: 1. Hemolytic streptococcal infections, such as pharyngitis, tonsillitis, scarlet fever, erysipelas, cellulitis and puerperal fever. 2. Pneumococcal infections such as pneumonia, otitis media, meningitis and bacteremia. 3. Non-penicillinase-producing staphylococcal infections. 4. Anthrax. 5. Clostridium infections such as tetanus and gas gangrene. 6. Syphilis (including congenital syphilis). 7. Leptospirosis. 8. Relapsing fever. 9. Diphtheria. 10. Penicillin is used in combination with aminoglycosides to treat viridans streptococcal endocarditis. Penicillin can also be used to treat: 1. Epidemic cerebrospinal meningitis. 2. Actinomycosis. 3. Gonorrhea. 4. Fensen angina. 5. Lyme disease. 6. Pasteurella multocida infection. 7. Rat-bite fever. 8. Listeria infection. 9. Many anaerobic infections except Bacteroides fragilis. Penicillin can be used to prevent infective endocarditis in patients with rheumatic heart disease or congenital heart disease before oral, dental, gastrointestinal or genitourinary tract surgery and procedures.
Usage and Dosage
Penicillin is administered by intramuscular injection or intravenous drip. 1 Adults: intramuscular injection, 800,000 to 2 million units per day, divided into 3 to 4 doses; intravenous drip, 2 million to 20 million units per day, divided into 2 to 4 doses. 2 Children: intramuscular injection, 25,000 units/kg of body weight, once every 12 hours; intravenous drip, 50,000 to 200,000 units/kg of body weight per day, divided into 2 to 4 doses. 3 Newborns (full-term): 50,000 units/kg of body weight each time, intramuscular injection or intravenous drip; once every 12 hours in the first week after birth, once every 8 hours for those over one week old, and once every 6 hours for severe infection. 4 Premature infants: 30,000 units/kg of body weight each time, once every 12 hours in the first week after birth, once every 8 hours for those 2 to 4 weeks old; once every 6 hours thereafter. 5 Patients with impaired renal function: Patients with mild and moderate renal impairment do not need to reduce the regular dose, while patients with severe renal impairment should extend the dosing interval or adjust the dose. When the endogenous creatinine clearance rate is 10-50 ml/min, the dosing interval is extended from 8 hours to 8-12 hours or the dosing interval remains unchanged and the dose is reduced by 25%; when the endogenous creatinine clearance rate is less than 10 ml/min, the dosing interval is extended to 12-18 hours or each dose is reduced to 25%-50% of the normal dose and the dosing interval remains unchanged. 6 For intramuscular injection, 500,000 units of penicillin sodium are dissolved in 1 ml of sterile water for injection. If it exceeds 500,000 units, 2 ml of sterile water for injection should be added. Sodium chloride injection should not be used as a solvent; the administration rate during intravenous drip should not exceed 500,000 units per minute to avoid central nervous system toxicity.
Adverse Reactions
1. Allergic reaction: Allergic reaction to penicillin is common, including urticaria and other types of rashes, leukopenia, interstitial nephritis, asthma attacks, and serum sickness reactions; anaphylactic shock is rare. Once it occurs, it must be rescued on the spot, and the airway must be kept open, oxygen inhalation, and treatment measures such as epinephrine and glucocorticoids must be used. 2. Toxic reaction: It is rare, but when a large dose of this product is intravenously infused or intrathecally administered, convulsions, muscle spasms, coma, and severe mental symptoms (penicillin encephalopathy) may occur due to excessive concentration of drugs in the cerebrospinal fluid. This reaction is more common in infants, the elderly, and patients with renal insufficiency. 3. Herxheimer's reaction and treatment contradiction: When penicillin is used to treat diseases such as syphilis and leptospirosis, symptoms may be aggravated due to the death of pathogens, which is called Herxheimer's reaction; treatment contradictions are also seen in syphilis patients. After treatment, the syphilis lesions disappear too quickly, while tissue repair is relatively slow or the fibrous tissue in the lesion site contracts, hindering organ function. 4. Dual infection: Dual infection with penicillin-resistant Staphylococcus aureus, Gram-negative bacilli or Candida albicans may occur. 5. The use of large doses of penicillin sodium may lead to heart failure due to the intake of large amounts of sodium salt.
Precautions
It is contraindicated in patients with a history of allergy to penicillins or a positive penicillin skin test.
Special Population Medication
Precautions for children: Not yet clear. Precautions for pregnancy and lactation: Animal reproduction tests have not found that this product causes fetal damage. However, strict controlled trials have not been conducted in pregnant women to exclude the adverse effects of this type of drug on the fetus, so pregnant women should only use this product when it is really necessary. A small amount of this product is secreted from breast milk, so breastfeeding should be stopped when breastfeeding women take the drug. Precautions for the elderly: Not yet clear.
Drug Interactions
1 Chloramphenicol, erythromycin, tetracyclines, and sulfonamides can interfere with the activity of this product, so this product should not be used in combination with these drugs. 2 Probenecid, aspirin, indomethacin, phenylbutazone, and sulfonamides reduce the renal tubular secretion of penicillin and prolong the serum half-life of this product. Penicillin can enhance the anticoagulant effect of warfarin. 3 This product is incompatible with heavy metals, especially copper, zinc, and mercury. 4 Adding lincomycin, tetracycline, vancomycin, erythromycin ethylsuccinate, amphotericin B, norepinephrine, metaraminol, phenytoin sodium, hydroxyzine hydrochloride, prochlorperazine, promethazine, vitamin B group, vitamin C group, etc. to penicillin intravenous infusion will cause turbidity. 5 This product and aminoglycoside antibiotics drip in the same bottle may lead to a decrease in the antibacterial activity of both, so they cannot be placed in the same container for administration.
