Function and Efficacy

Acyclovir has an inhibitory effect on herpes simplex virus, varicella zoster virus, cytomegalovirus, etc. in vitro. The drug is easily taken up by herpes simplex virus, then phosphorylated into triphosphate, which inhibits viral replication in two ways: interfering with viral DNA polymerase and inhibiting viral replication; under the action of DNA polymerase, it binds to the growing DNA chain, causing the extension of the DNA chain to be interrupted.

Appearance

This product is a colorless or almost colorless clear liquid.

Indication

1. Herpes simplex virus infection: used for the treatment of primary and recurrent mucocutaneous infections in immunocompromised individuals and the prevention of recurrent cases; also used for the treatment of herpes simplex encephalitis. 2. Herpes zoster: used for the treatment of severe herpes zoster in immunocompromised individuals or disseminated herpes zoster in immunocompetent individuals. 3. Treatment of varicella in immunocompromised individuals.

Usage and Dosage

This product is diluted to at least 100ml with 0.9% saline or 5% glucose water, so that the final drug concentration does not exceed 7g/L, otherwise it is easy to cause phlebitis. Intravenous drip: 1 Adult ① Initial treatment of severe genital herpes: 5mg/kg per body weight every 8 hours for 5 days; ② Skin and mucous membrane herpes simplex or severe herpes zoster in immunocompromised patients: 5-10mg/kg per body weight every 8 hours, intravenous drip for more than 1 hour, for 7-10 days; ③ Herpes simplex encephalitis: 10mg/kg per body weight every 8 hours for 10 days; ④ Adults with acute or chronic renal insufficiency should not use this product for intravenous drip, because too fast a drip rate can cause renal failure; ⑤ The maximum daily dose for adults is 30mg/kg per body weight, or 1.5g/m2 per body area. 2 Children ① Initial treatment of severe genital herpes: infants and children under 12 years old, 250 mg/m2 of body surface area every 8 hours for 5 days; ② Herpes simplex on the skin and mucous membranes in immunocompromised patients: infants and children under 12 years old, 250 mg/m2 of body surface area every 8 hours for 7 days, and those over 12 years old should use the adult dose; ③ Herpes simplex encephalitis: 10 mg/kg of body weight every 8 hours for 10 days; ④ Immunodeficiency patients with varicella: 10 mg/kg or 500 mg/m2 every 8 hours for 10 days; ⑤ The maximum dose for children is 500 mg/m2 of body surface area every 8 hours.

Adverse Reactions

1. Common adverse reactions: If the injection concentration is too high (10g/L), it may cause phlebitis, and inflammation may occur at the injection site when overflow occurs. It may also cause skin itching or urticaria. 2. Rare adverse reactions: When injecting, especially intravenous injection, there may be acute renal insufficiency, hematuria and hypotension. 3. Rare adverse reactions: When injecting, central nervous system symptoms such as coma, confusion, hallucinations, and epilepsy may occur. 4. The following symptoms should be paid attention to if they persist or are obvious: mild headache caused by injection (common), sweating (rare).

Precautions

Special Population Medication

Precautions for children: Although no special adverse reactions have been found in children, it should be used with caution. It is not advisable for newborns to prepare infusion solutions with diluents containing benzyl alcohol, otherwise it is easy to cause fatal syndromes, including acidosis, central nervous system depression, dyspnea, renal failure, hypotension, epilepsy and intracranial hemorrhage. Precautions for pregnancy and lactation: Use with caution in pregnant women. From 1984 to April 1994, an epidemiological record on the efficacy of acyclovir in pregnant women was established. Among the 756 experimental results for a period of 3 months, 749 pregnant women successively took acyclovir systematically, and its teratogenic effect on infants was similar to that of the general population. However, these data are not enough to prove that acyclovir is safe for pregnant women and fetuses. Acyclovir can only be considered when the therapeutic effect of acyclovir on the fetus is far greater than its risk. The concentration of acyclovir in breast milk is 0.6 to 4.1 times the blood concentration. When the dosage of a lactating woman reaches 0.3 mg/kg per day, it may affect the development of the baby. Acyclovir can only be used by breastfeeding women when necessary. Elderly precautions: There is currently no sufficient research data to show that the use of drugs for elderly people over 65 years old is significantly different from that for young people. In general, elderly people should be cautious when taking drugs, choose a low dose within the effective range, and try to reduce the occurrence of renal function impairment or other side effects caused by increasing the number of medications.

Drug Interactions

1. When used intravenously with interferon or methotrexate (intrathecal), it may cause mental abnormalities and should be used with caution. 2. When used intravenously with nephrotoxic drugs, it may aggravate nephrotoxicity, especially in patients with renal insufficiency. 3. When used in combination with zidovudine, it may cause nephrotoxicity, manifested as deep lethargy and fatigue.
