Glimepiride Dispersible Tablets

Function and Efficacy

The active ingredient of this product is glimepiride, which is an oral sulfonylurea hypoglycemic drug. It is used to treat non-insulin-dependent diabetes mellitus. Glimepiride mainly works by stimulating pancreatic β cells to release insulin. Like other sulfonylurea drugs, the effect of glimepiride on pancreatic β cells to release insulin is mainly based on increasing the responsiveness of pancreatic β cells to physiological concentrations of glucose. In addition, glimepiride also has obvious effects outside the pancreas, which also exist in other sulfonylurea drugs. Insulin release: Sulfonylurea drugs regulate insulin secretion by closing the ATP-dependent potassium channels of pancreatic β cell membranes. Closing potassium channels induces β cell membrane depolarization, while opening calcium channels leads to increased calcium ion influx into cells, stimulating insulin release through exocytosis. Glimepiride binds to β cell membrane proteins associated with ATP-dependent potassium channels at a high exchange rate, and its binding site is different from the binding site of traditional sulfonylurea drugs. Extrapancreatic activity: Extrapancreatic effects improve the sensitivity of peripheral tissues to insulin and reduce the uptake of insulin by the liver. Peripheral muscle and adipose tissue take up glucose from the blood through specific transporters located in the cell membrane. The transfer of glucose in these tissues limits the rate of glucose utilization. Glimepiride rapidly increases the number of active glucose transfer molecules in the plasma membrane of muscle and adipocytes, thereby stimulating glucose uptake. Glimepiride increases the activity of glycosyl-phosphatidylinositol-specific phosphatidic acid C, which is related to the drug-induced synthesis of fat and glycogen in isolated fat and muscle cells. Glimepiride inhibits the production of glucose in the liver by increasing the intracellular concentration of fructose 2,6-bisphosphate, thereby inhibiting the formation of glycogen. General pharmacodynamics: The minimum effective oral dose in healthy individuals is about 0.6 mg. The effect of glimepiride is dose-dependent and reproducible. The physiological response of reduced insulin secretion during intense exercise still exists when taking glimepiride. There is no significant difference in the therapeutic effect whether the drug is administered 30 minutes before meals or immediately before meals. A single daily dose can well control the metabolism of diabetic patients for 24 hours. Although the hydroxy metabolites of glimepiride cause a small but statistically significant decrease in serum glucose in healthy subjects, this is only a small part of the overall effect of the drug. Combination therapy with insulin: There is limited data on combination therapy with insulin. When patients still cannot effectively control blood sugar with the maximum dose of glimepiride, combined insulin therapy can be started. Two studies have confirmed that combination therapy can achieve the same metabolic control as insulin therapy alone; however, the average dose of insulin required for combination therapy is lower. Indication

Usage and Dosage

Take the medication orally as directed by your doctor. For patients with diabetes, there is no fixed dose for glimepiride or any other glucose-lowering drug. Fasting blood glucose and glycated hemoglobin must be measured regularly to determine the minimum effective dose of the patient's medication; glycated hemoglobin levels must be measured to monitor the patient's treatment effect. Usual starting dose: In the initial treatment stage, the starting dose of glimepiride is 1-2 mg once a day, administered at breakfast or the first main meal. Those patients who are sensitive to glucose-lowering drugs should start with 1 mg once a day, and the dose should be adjusted with caution. Adverse Reactions

Systemic disorders Occasionally, allergic or pseudoallergic reactions such as itching, urticaria or rash may occur. These mild reactions may develop into severe reactions with difficulty breathing and decreased blood pressure, sometimes to shock. If urticaria occurs, the doctor must be notified immediately. Special Population Medication

Precautions for children: There is a lack of research data on the safety and efficacy of this product for children. An experiment evaluating the pharmacokinetics, safety and tolerability of a single dose of 1 mg of glimepiride in 30 pediatric type 2 diabetes patients (aged 10-17 years) showed that the average AUC (0-last), Cmax and t1/2 were similar to those previously observed in adults. Precautions for pregnancy and lactation: Pregnancy Glimepiride tablets are contraindicated during pregnancy. Otherwise, there is a risk of harming the fetus. Pregnant patients must switch to insulin.
