Praziquantel Tablets

Function and Efficacy

This product is effective against schistosomes, tapeworms, cysticercosis, Clonorchis sinensis, Paragonimus thoracis, and Fasciola gingivalis. It has two main pharmacological effects on the worm: 1. The muscles of the worm undergo tonic contraction and produce spastic paralysis. After the schistosomes are exposed to low concentrations of praziquantel, the tension of the worm increases in just 20 seconds. When the drug concentration reaches 1 mg/L or more, the worm instantly and strongly contracts. The muscle contraction of the worm may be related to the fact that praziquantel increases the permeability of the worm cell membrane and causes the loss of intracellular calcium ions. 2. Damage to the cortex of the worm and the participation of the host immune function: Praziquantel has a rapid and obvious damage effect on the cortex of the worm, causing the syncytial epidermis to swell, vacuoles to appear, bullae to form, protruding from the body surface, and finally the epidermis erodes and ruptures, the secretory body almost disappears, and the circular muscle and longitudinal muscle also dissolve rapidly and successively. In the host, vacuolar degeneration of the worm epidermis can be seen 15 minutes after taking the drug. After the cortex is damaged, the absorption and excretion functions of the worm are affected. More importantly, its surface antigens are exposed, making it vulnerable to the host's immune attack. A large number of eosinophils attach to the lesions and invade, causing the worm to die. In addition, praziquantel can also cause secondary changes, depolarizing the worm's surface membrane, significantly reducing the activity of cortical alkaline phosphatase, inhibiting glucose uptake, and depleting endogenous glycogen. Praziquantel can also inhibit the synthesis of nucleic acid and protein in the worm.

Ingredients

Chemical name: 2-cyclohexylcarbonyl-1,2,3,6,7,11b-hexahydro-4H-pyrazino[2,1-α]isoquinolin-4-one, molecular formula: C19H24N2O2, molecular weight: 312.41.

Indication

It is a broad-spectrum drug against trematodes and tapeworms. It is suitable for various schistosomiasis, clonorchiasis, paragonimiasis, fasciolosis, tapeworm disease and cysticercosis.

Usage and Dosage

1 Treatment of Trematodiasis ① Schistosomiasis: Various chronic schistosomiasis adopts a 1-2 day therapy with a total dose of 60mg/kg, which is taken 2-3 times a day between meals. The total dose for acute schistosomiasis is 120mg/kg, which is taken 2-3 times a day for 4 consecutive days. Those weighing more than 60kg are calculated as 60kg. ② Clonorchiasis: The total dose is 210mg/kg, 3 times a day, for 3 consecutive days. ③ Paragonimiasis: 25mg/kg, 3 times a day, for 3 consecutive days. ④ Fasciola fasciolae: 15mg/kg, taken all at once. 2 Treatment of Tapeworms ① Beef and pork tapeworms: 10mg/kg, taken all at once in the morning, and magnesium sulfate is taken 1 hour later. ② Hymenolepidium brevis and Diphyllobothrium latum: 25mg/kg, taken all at once. 3. The total dose for the treatment of cysticercosis is 120-180 mg/kg, taken for 3-5 days, and the daily dose is taken in 2-3 times.

Adverse Reactions

1 Common side effects include dizziness, headache, nausea, abdominal pain, diarrhea, fatigue, limb pain, etc., which are generally mild and short-lasting, do not affect treatment, and do not require treatment. 2 A few cases have symptoms such as palpitations and chest tightness. The electrocardiogram shows T wave changes and extrasystoles, and occasionally supraventricular tachycardia and atrial fibrillation. 3 A few cases may have transient elevation of transaminase. 4 Occasionally, it may induce mental disorders or gastrointestinal bleeding.

Precautions

1. It is forbidden for patients with ocular cysticercosis. 2. Breastfeeding women should not breastfeed during the medication period or within 72 hours after stopping the medication.
