Function and Efficacy

This product is a bio-ester base with anti-cancer effect extracted from Cephalotaxus plants. It can inhibit the synthesis of eukaryotic cell proteins, disaggregate polyribosomes, and interfere with the function of protein nucleosome sugars. This product also has an inhibitory effect on the synthesis of DNA in cells. In vitro experiments have shown that this product has the strongest killing effect on G1 and G2 phase cells, but has a smaller effect on S phase cells. This product has no cross-resistance with cytarabine, mercaptopurine, etc.

Appearance

This product is a colorless clear liquid.

Indication

It is suitable for various types of acute non-lymphocytic leukemia, and also has certain therapeutic effects on myelodysplastic syndrome (MDS), chronic myeloid leukemia and polycythemia vera.

Usage and Dosage

(1) The common dose for adults is intravenous drip, 1-4 mg per day, add 250-500 ml of 5% glucose injection, slowly drip over 3 hours, 4-6 days as a course of treatment, rest for 1-2 weeks and repeat the medication. (2) The common dose for children is intravenous drip, 0.05-0.1 mg/kg per day according to body weight, 4-6 days as a course of treatment.

Adverse Reactions

(1) Bone marrow suppression This product has an inhibitory effect on all series of hematopoietic cells in the bone marrow. The inhibition of the granulocyte series is more severe, followed by the erythrocyte series, and the inhibition of the megakaryocyte series is less severe. (2) Cardiac toxicity The more common cardiac toxicity includes sinus tachycardia, atrial or ventricular extrasystoles, and myocardial ischemia manifestations such as S-T segment changes and T wave flatness on the electrocardiogram. A very small number of patients may experience gallop rhythm, varying degrees of atrioventricular block and bundle branch block, atrial fibrillation, etc. (3) Hypotension Literature reports that when the dose of homoharringtonine is greater than 3.0 mg/m2 per dose, some patients will experience a decrease in blood pressure about 4 hours after administration. (4) Common symptoms of the digestive system are anorexia, nausea, and vomiting. A few patients may develop liver damage. (5) Some patients may experience hair loss and rash. Occasionally, a case of suspected severe anaphylactic shock has been reported.

Drug Interactions

(1) The dosage and course of treatment of this product should be adjusted when it is used in combination with other anticancer drugs or radiotherapy that may inhibit bone marrow function. (2) Anthracycline antibiotics have myocardial toxicity. Elderly patients and patients who have been repeatedly treated with anthracycline antibiotics such as doxorubicin or daunorubicin should use harringtonine with caution or not at all to avoid increased cardiotoxicity.
