Isosorbide Mononitrate for Injection
Function and Efficacy
Isosorbide mononitrate (ISMN) is the main biologically active metabolite of isosorbide dinitrate. Like other organic nitrates, its main pharmacological action is to relax vascular smooth muscle. ISMN releases nitric oxide (NO). NO is the same as endothelial relaxing factor, activating guanylate cyclase, increasing cyclic guanosine monophosphate (cGMP) in smooth muscle cells, thereby relaxing vascular smooth muscle, dilating peripheral arteries and veins, and having a stronger dilating effect on veins. Venous dilation causes blood to accumulate in the periphery, reducing the amount of blood returning to the heart, and reducing left ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload). Arterial dilation reduces peripheral vascular resistance, systolic arterial pressure, and mean arterial pressure (afterload). Coronary artery dilation increases coronary perfusion. The overall effect is to reduce myocardial oxygen consumption and increase oxygen supply.

Appearance
This product is a colorless clear liquid.
Indication
It is suitable for the treatment of angina pectoris and can be used in combination with digitalis and (or) diuretics to treat chronic heart failure.
Usage and Dosage
Intravenous drip. The drug dosage can be adjusted according to the patient's response. The general effective dose is 2-7 mg per hour. The initial administration rate is 60 μg/min, and the general rate is 60-120 μg/min, once a day, and a course of treatment is 10 days.
Adverse Reactions
Nitrate-induced vasodilatory headaches may occur in the early stages of medication, but usually disappear after a few days of continuous use. Facial flushing, dizziness, orthostatic hypotension, and reflex tachycardia may also occur. Occasionally, blood pressure may drop significantly, bradycardia, aggravated angina pectoris, and syncope may occur.
Precautions
Acute circulatory failure (shock, circulatory collapse); severe hypotension (systolic blood pressure < 90 mmHg); acute myocardial infarction with low filling pressures (unless under continuous hemodynamic monitoring); hypertrophic obstructive cardiomyopathy; constrictive pericarditis or cardiac tamponade; severe anemia; glaucoma; increased intracranial pressure; allergy to nitro compounds
Drug Interactions
Combination with other vasodilators, calcium antagonists, beta-blockers, antihypertensive drugs, tricyclic antidepressants and alcohol can enhance the blood pressure lowering effect of this class of drugs.
