    Description

    Sirolimus, also known as rapamycin, is a natural macrocyclic lactone produced by the bacterium Streptomyces hygroscopicus, with immunosuppressant properties. In cells, sirolimus binds to the immunophilin FK Binding Protein-12 (FKBP-12) to generate an immunosuppressive complex that binds to and inhibits the activation of the mammalian Target Of Rapamycin (mTOR), a key regulatory kinase. This results in inhibition of T lymphocyte activation and proliferation that occurs in response to antigenic and cytokine (IL-2, IL-4, and IL-15) stimulation and inhibition of antibody production.

    Sirolimus, also known as rapamycin, is an immunosuppressant drug that is primarily used to prevent organ transplant rejection and to treat certain rare diseases. It is classified as a macrolide antibiotic and is notable for its unique mechanism of action that differentiates it from other immunosuppressants.

    Mechanism of Action

    Sirolimus exerts its effects by inhibiting the mammalian target of rapamycin (mTOR), a crucial serine/threonine kinase involved in regulating cell growth, proliferation, and survival. The mechanism involves:
    Binding to FKBP12: Sirolimus forms a complex with the intracellular protein FK506-binding protein 12 (FKBP12). This complex then binds to mTOR, inhibiting its activation.
    Inhibition of T-Cell Proliferation: By inhibiting mTOR, sirolimus disrupts the signaling pathways that are essential for T-cell activation and proliferation in response to cytokines such as interleukin-2 (IL-2). This prevents T-cells from progressing from the G1 phase to the S phase of the cell cycle.
    Antibody Production Suppression: Sirolimus also inhibits B-cell activation and antibody production, further contributing to its immunosuppressive effects.

    Applications

    Sirolimus is indicated for:
    Organ Transplantation: Primarily used to prevent rejection in kidney transplants, often in combination with other immunosuppressants like calcineurin inhibitors.
    Lymphangioleiomyomatosis (LAM): A rare lung disease characterized by abnormal smooth muscle cell proliferation.
    Perivascular Epithelioid Cell Tumor (PEComa): Approved for treatment in adults with locally advanced unresectable or metastatic PEComa.
    Cardiovascular Applications: Used as a coating on drug-eluting stents to prevent restenosis by inhibiting smooth muscle cell proliferation.

    Routes of Administration

    Sirolimus is administered orally. The typical dosing regimen includes:
    Initial Dose: An initial loading dose of 6 mg is often given.
    Maintenance Dose: Followed by a maintenance dose of 2 mg daily.

    Safety and Side Effects

    While sirolimus is generally well-tolerated, it can cause several side effects:
    Common Side Effects: Hyperlipidemia (increased cholesterol and triglycerides), thrombocytopenia (low platelet count), anemia, and leukopenia (low white blood cell count).

    Elimination: The elimination half-life ranges from 30 to 60 hours. It is excreted mainly through feces (approximately 90%), with renal excretion accounting for about 5–10%.
