Propitocaine hydrochloride, more commonly known as prilocaine hydrochloride, is an amide-type local anesthetic used primarily for local and regional anesthesia.

Mechanism of Action

Propitocaine hydrochloride acts by stabilizing neuronal membranes through inhibition of sodium ion influx via voltage-gated sodium channels on nerve cells. This blockade prevents the initiation and propagation of nerve impulses, thereby producing local anesthesia and reducing pain sensation.

Clinical Applications and Routes of Administration 

Local infiltration anesthesia: Used in dental procedures, minor surgeries, and skin interventions.
Nerve blocks: Employed for regional anesthesia in various clinical settings.
Topical anesthesia: Often combined with lidocaine (e.g., in EMLA cream) for dermal analgesia prior to needle insertion or minor skin surgeries.
Intravenous regional anesthesia (IVRA): Due to its relatively low systemic toxicity, prilocaine is favored for IVRA.
Administration routes include subcutaneous or infiltration injection and topical application on skin or mucous membranes.

Safety and Side Effects

Methemoglobinemia: A significant risk due to o-toluidine metabolite, which can impair oxygen transport. Symptoms include cyanosis, headache, dizziness, and fatigue. This condition is dose-dependent and reversible with intravenous methylene blue.
Central nervous system effects: Dizziness, excitability, seizures (rare).
Cardiovascular effects: Potential changes in heart rate, blood pressure; generally low cardiac toxicity but caution advised.
Allergic reactions: Rare but may include rash, urticaria, anaphylaxis.
Other side effects: Numbness, tingling, blurred vision, nausea, and local irritation.
Precautions: Use cautiously in patients with liver or kidney dysfunction, anemia, or predisposition to methemoglobinemia.

Pharmacokinetics

Absorption: Rapid and complete absorption after parenteral administration; rate depends on site and presence of vasoconstrictors.
Distribution: Approximately 55% bound to plasma proteins; crosses blood-brain and placental barriers by passive diffusion.
Metabolism: Metabolized mainly in the liver and kidneys by amidases to o-toluidine and N-propylalanine. O-toluidine can induce methemoglobinemia.
Excretion: Primarily renal elimination of metabolites; unchanged drug excretion is minimal.
Half-life: Variable, generally around 70 minutes; may be prolonged in hepatic or renal impairment.
