Losartan potassium is an orally active antihypertensive drug belonging to the angiotensin II receptor blocker (ARB) class. It is widely used to treat high blood pressure, reduce the risk of stroke, protect kidneys in diabetic nephropathy, and manage certain cardiovascular conditions.

Mechanism of Action

Losartan selectively and competitively blocks the binding of angiotensin II to the AT1 receptor found in vascular smooth muscle and adrenal glands. By inhibiting this receptor:
It prevents angiotensin II–mediated vasoconstriction, leading to vasodilation and lowered blood pressure.
It reduces aldosterone secretion, promoting sodium and water excretion, which further decreases blood volume and pressure.
This blockade helps reduce cardiac workload and prevents pathological remodeling associated with hypertension and heart failure.

Applications

Hypertension: Used to lower high blood pressure in adults and children aged 6 years and older.
Stroke Risk Reduction: Decreases risk of stroke in hypertensive patients with left ventricular hypertrophy (though this benefit may be less in patients of African descent).
Diabetic Nephropathy: Protects kidneys by reducing proteinuria and slowing progression of kidney damage in type 2 diabetic patients with hypertension.
Heart Failure: Used to improve survival and manage symptoms in heart failure patients.
Other Uses: Sometimes used in Marfan syndrome and other cardiovascular risk management plans.
Losartan is also available in fixed-dose combination with hydrochlorothiazide for enhanced antihypertensive effect.

Routes of Administration

Oral: Administered as tablets or oral suspension without regard to meals.

Pharmacokinetics

Absorption: Losartan is well absorbed orally but undergoes substantial first-pass metabolism, resulting in about 33% systemic bioavailability.
Peak Concentrations: Losartan peaks in plasma approximately 1 hour after dosing; its active metabolite peaks at 3–4 hours.
Metabolism: Metabolized primarily by CYP2C9 and CYP3A4 enzymes into an active carboxylic acid metabolite responsible for much of its therapeutic effect.
Half-life: Losartan has a terminal half-life of about 2 hours; its active metabolite’s half-life ranges from 6 to 9 hours.
Elimination: Excreted via urine (~35%) and feces (~60%).
Special Populations: In hepatic impairment, bioavailability and plasma concentrations increase; dosage adjustment is recommended. Renal impairment increases plasma levels but usually does not require dosage change unless volume depleted.

Safety and Side Effects

Common Side Effects: Dizziness, fatigue, upper respiratory infections, cough (less frequent than with ACE inhibitors), headache, nausea, diarrhea, back pain, and mild hyperkalemia.
Serious Side Effects: Rare but include kidney damage, severe hypotension, angioedema (swelling of face, neck, mouth), allergic reactions, and hyperkalemia which may be life-threatening.
Contraindications: Pregnancy (FDA category D) due to risk of fetal toxicity; avoid use during pregnancy and breastfeeding.
Precautions: Monitor blood pressure, renal function, and serum potassium periodically. Use cautiously in patients with volume depletion, renal or hepatic impairment.
Drug Interactions: May interact with potassium supplements, potassium-sparing diuretics, NSAIDs, and other antihypertensives.
