Milrinone is an intravenous medication used for the short-term treatment of acute decompensated heart failure (ADHF) in a hospital or intensive care setting. It is an "ino-dilator," meaning it increases the heart's contractility and causes blood vessel dilation.

Mechanism of action

Milrinone is a selective phosphodiesterase-3 (PDE-3) inhibitor. This action increases the level of cyclic adenosine monophosphate (cAMP) in cardiac and vascular muscle cells, which in turn increases intracellular calcium levels in heart muscle (enhancing the force of contraction, or positive inotropy) and decreases calcium in vascular smooth muscle (causing relaxation and vasodilation).

It has a role as an EC 3.1.4.17 (3',5'-cyclic-nucleotide phosphodiesterase) inhibitor, a platelet aggregation inhibitor, a vasodilator agent and a cardiotonic drug.
