   Cagrilintide is a new generation long-acting injectable amylin analog, whose mechanism of action is different from the currently approved GLP-1-based weight loss therapies. These results represent the first and also the only Phase I clinical trial data for the treatment of obesity with an investigational long-acting amylin analog as a monotherapy.It is a single peptide combined with a fatty diacid, consisting of 39 amino acids. It acts as an agonist for glucagon-like peptide-1 (GLP-1), glucose-dependent insulinotropic polypeptide (GIP), and glucagon (GCGR) receptors. Compared with endogenous ligands, Rytarutide has weaker effects on human GCGR and GLP-1 receptors, but stronger effects on human GIP receptors. Its structure contains a fatty acid component, which enhances its stability in the body, prolongs its duration of action, and allows it to be administered once weekly.

Mechanism of action: Cagrilintide is a glucagon-like peptide-1 (GLP-1) receptor agonist. It binds to GLP-1 receptors, activating downstream G protein-coupled signaling pathways, mainly including the adenylyl cyclase (AC)-cyclic adenosine monophosphate (cAMP)-protein kinase A (PKA) pathway. This process promotes insulin secretion by pancreatic β cells when blood glucose levels rise, while inhibiting glucagon secretion by pancreatic α cells, thereby lowering blood glucose levels. In addition, Cagrilintide can delay gastric emptying, reduce the rate of food absorption, and lower postprandial blood glucose peaks; it also acts on the central nervous system to reduce appetite and increase satiety, helping with weight control.

Research Progress: In terms of clinical research, Cagrilintide has conducted multiple clinical trials targeting type 2 diabetes. Some studies indicate that it can effectively reduce patients' glycated hemoglobin (HbA1c) levels and improve blood sugar control. In terms of safety, common adverse reactions include gastrointestinal responses such as nausea and vomiting, but most are mild to moderate and generally tolerable for patients. In addition to type 2 diabetes, studies have also explored its potential in the treatment of obesity, and preliminary results show some effect in weight reduction. However, Cagrilintide is still in the development stage, and more large-scale, long-term clinical trials are needed to further evaluate its efficacy and safety in order to determine its role in clinical treatment.
