High Purity Orforglipron Oral GLP-1 Agonist for Blood Sugar & Weight Management

Orforglipron is a synthetic small-molecule compound classified as a non-peptide glucagon-like peptide-1 receptor agonist. Its molecular formula is C₄₈H₄₈F₂N₁₀O₅, with a molecular weight of approximately 882.96 Da, and it appears as an off-white to pale yellow solid powder. It is soluble in DMSO but insoluble in water and ethanol, and must be stored at -20°C protected from light. As an orally active non-peptide GLP-1R agonist, Orforglipron exhibits a different binding mode to the receptor compared to natural GLP-1 peptides. High-resolution cryo-EM structures reveal that its binding site is located in a unique pocket on the upper part of the receptor’s helical bundle, involving the extracellular domain, extracellular loop 2, and transmembrane helices 1, 2, 3, and 7. Pharmacologically, it acts as a partial agonist (relative to native GLP-1), with signaling biased toward G protein activation and no β-inhibitory protein recruitment activity. Its binding affinity (K_i) for the human GLP-1R is 3.22 nM. It exhibits high oral bioavailability and favorable pharmacokinetic properties. Unlike the oral semaglutide, Orforglipron is not subject to food or water restrictions, can be taken at any time of day, and does not require refrigeration. Its XLogP value is 7.77, indicating high lipophilicity.

(1) Obesity Studies: For chronic weight management in adults with obesity (BMI ≥ 30 kg/m²) or overweight (BMI ≥ 27 kg/m² with at least one weight-related comorbidity). The Phase III clinical trial ATTAIN-1 evaluated the efficacy and safety of Orforglipron (6 mg, 12 mg, 36 mg, once daily) in 3,127 adults with obesity or overweight who did not have diabetes.

(2) Type 2 Diabetes Studies: For improving glycemic control in adults with type 2 diabetes. The Phase III clinical trial ATTAIN-2 evaluated the efficacy and safety of Orforglipron in 1,613 obese or overweight adults with type 2 diabetes. Another Phase III trial, ACHIEVE-1, assessed its hypoglycemic effects in patients with early-stage type 2 diabetes.

(3) Cardiometabolic Disease Studies: Clinical trials are evaluating its potential therapeutic effects on comorbidities such as obstructive sleep apnea, hypertension, and knee osteoarthritis in obese or overweight adults.

(1) Significant weight loss: In obese patients without diabetes, treatment with 36 mg of Orforglipron for 72 weeks resulted in an average weight reduction of 11.2%, compared with a reduction of only 2.1% in the placebo group. Among these patients, 54.6% lost ≥10% of their body weight, 36.0% lost ≥15%, and 18.4% lost ≥20%. In obese patients with type 2 diabetes, 72 weeks of treatment with Orforglipron 36 mg resulted in an average weight loss of 9.6% (approximately 9.6 kg), compared to 2.5% (approximately 2.7 kg) in the placebo group. Data from Chinese patients showed that the highest dose group achieved an average weight loss of 12.4 kg (12.4%).

(2) Effective improvement in glycemic control: In obese patients with type 2 diabetes, treatment with Orforglipron 36 mg for 72 weeks resulted in a 1.7% reduction in glycated hemoglobin (HbA1c) (from a baseline of 8.1% to 6.4%), compared with a 0.5% reduction in the placebo group. 75.5% of patients achieved HbA1c < 7%, and 66.6% achieved HbA1c ≤ 6.5%. In patients with early-stage type 2 diabetes, 40 weeks of treatment with Orforglipron 36 mg reduced HbA1c by 1.48% (from 8.0% at baseline to 6.5%), compared to a 0.41% reduction in the placebo group.

(3) Improvement in Cardiometabolic Parameters: Orforglipron significantly improved multiple cardiovascular risk factors. These included: reduced waist circumference (mean reduction of 9.2 cm in the 36 mg group vs. 2.7 cm in the placebo group); decreased systolic blood pressure; reduced non-HDL cholesterol and triglyceride levels; and a 50.6% reduction in high-sensitivity C-reactive protein levels (in the 36 mg group), suggesting an anti-inflammatory effect.

(4) Oral Convenience and Accessibility: As a non-peptide small molecule, Orforglipron offers the advantages of high oral bioavailability, no dietary or fluid intake restrictions, and no need for cold-chain storage or transportation, thereby improving patient adherence and drug accessibility. Its scalability and portability are expected to meet the growing global demand for obesity and diabetes treatment.

(5) Safety Profile: The overall safety profile of Orforglipron is consistent with that of GLP-1 receptor agonists. The most common adverse events were mild to moderate gastrointestinal reactions (nausea, diarrhea, vomiting, constipation), primarily occurring during the dose-escalation phase. In the 36 mg group, the incidence rates were 36.4% for nausea, 23.1% for vomiting, and 27.4% for diarrhea. The discontinuation rate due to adverse events ranged from 6.1% to 10.9%, compared to 4.6% in the placebo group. No safety signals related to liver function were observed.
