Rotundine Natural Alkaloid for Pain Relief, Sedation and Sleep Improvement

1. Description

Rotundine, also known as Cranial Pain Reliever or Levotetraspam, has the chemical name 2,3,9,10-tetramethoxy-5,8,13,13a-tetrahydro-6H-dibenz [a,g]quinoline. It is the levorotatory isomer of Corydaline (dl-THP) and acts as a dopamine receptor antagonist in the brain. It preferentially blocks dopamine receptors in brain regions rich in these receptors, such as the cortex and striatum, and exerts its analgesic effect by enhancing the function of the endogenous pain-inhibitory system and suppressing the transmission of pain signals to the central nervous system. Its analgesic effect is weaker than that but stronger than that of general analgesics. At therapeutic doses, it does not cause respiratory depression or gastrointestinal smooth muscle spasm, and is effective for acute and chronic persistent pain as well as visceral dull pain. First launched in China in 1985, it is indicated for patients with insomnia caused by pain, as well as for pain associated with gastric and duodenal ulcers, dysmenorrhea, postpartum uterine contractions, tension-type insomnia, and spasmodic cough.

2. Application

(1) Clinical analgesic studies: Used to treat visceral pain caused by gastrointestinal disorders, such as pain from gastric and duodenal ulcers (particularly effective when accompanied by gastrointestinal spasm); also used for general headaches, neuralgic headaches, dysmenorrhea, postpartum uterine contractions, and menstrual cramps. The commonly used clinical dose is 60–120 mg per dose orally or 60–90 mg per dose intramuscularly, with onset of action occurring in approximately 15–30 minutes.

(2) Sedative and Hypnotic Studies: Used in studies on sedation and hypnotic effects in patients with tension-related pain or insomnia caused by pain. While producing analgesic effects, rotundin can induce physiological sleep characterized by faster sleep onset and deeper sleep, with no residual effects upon awakening.

(3) Cardiovascular Effects: Studies indicate that rotundin possesses antiarrhythmic properties and exerts a protective effect in animal models of arrhythmias induced by , barium chloride, calcium chloride, and coronary artery ligation-reperfusion. It also exhibits hypotensive effects, reducing peripheral vascular resistance and left ventricular end-diastolic pressure.

(4) Research Applications: Used as an analytical standard for research purposes such as quantitative analysis, pharmacological experiments, and bioactivity screening.

3. Main Efficacy

(1) Analgesic Effects: It exhibits moderate analgesic activity and is particularly effective for chronic persistent pain and visceral dull pain, though its efficacy is slightly lower for acute sharp pain. Its mechanism of action is associated with the blockade of dopamine D1 and D2 receptors in the brain and the inhibition of the ascending reticular activating system in the brainstem; it is unrelated to the receptor system and therefore does not cause addiction or tolerance. In mouse hot-plate and writhing tests, rotundine significantly increased the pain threshold.

(2) Sedative and Hypnotic Effects: While producing analgesic effects, it also induces sedation and sleep, making it particularly suitable for cases of insomnia caused by pain. Its sedative mechanism involves inhibiting the activity of dopaminergic neurons in the mesolimbic system and the hypothalamus; it does not suppress the respiratory center and has minimal impact on normal sleep architecture. Clinical studies indicate that rotundine shortens sleep latency, prolongs total sleep time, and reduces the number of awakenings.

(3) Antiarrhythmic Effects: In various animal models of arrhythmia, rotundine can counteract arrhythmias induced by barium chloride, calcium chloride, and coronary ligation-reperfusion. The mechanism of action may be related to the inhibition of sodium and calcium channels in cardiac myocytes.

(4) Hypotensive Effects: Intravenous administration of rotundine causes a dose-dependent decrease in blood pressure in anesthetized animals; the mechanism involves blocking α-adrenergic receptors and inhibiting calcium release in vascular smooth muscle cells.

(5) Non-addictive and non-respiratory depressant: It is non-addictive at therapeutic doses and does not produce drug dependence with long-term use. Unlike analgesics, rotundine does not depress the respiratory center at analgesic doses and has no significant effect on ventilatory function. It also does not cause gastrointestinal smooth muscle spasm, making it well-suited for patients with constipation.
