In endothelial cells, nitric oxide (NO) activates guanylate cyclase (GC) to catalyze GTP and produce cGMP. Therefore, the relaxation of vascular smooth muscle is induced.

PDE5 inhibitors’ core mechanism is to inhibit the activity of PDE5 enzymes to prevent CGMP’s degradation, thereby the action of CGMP is enhanced and prolonged to increase the concentration of CGMP in smooth muscle cells and promote the relaxation of vascular smooth muscle and the blood flow. PDE5 inhibitors can enhance the relaxation effect of smooth muscle in the penis’s corpus cavernosum, and promote blood flow.
In addition to its role within the penile sponge, PDE5 inhibitors are also used to treat pulmonary arterial hypertension. This type of drug reduces PDE5 activity in pulmonary blood vessels, increases CGMP levels, relax vascular smooth muscle, and then reduce pulmonary artery pressure to improve blood flow.
Clinical Application
PDE5 inhibitors’ most well-known application is to treat male erectile dysfunction. Sildenafil, Tadalafil, and Vardenafil are common PDE5 inhibitors. By inhibiting PDE5, they can significantly improve erectile function under sexual stimulation. They have been widely used in patients with mild, moderate, and severe erectile dysfunction.
PDE5 inhibitors are also used to treat pulmonary arterial hypertension, a serious cardiovascular disease caused by high pulmonary artery pressure, which is manifested as an increase in pulmonary artery pressure and excessive right ventricular burden. PDE5 inhibitors can reduce vascular resistance by dilating pulmonary blood vessels. The burden on the heart will be reduced and patients' exercise ability and quality of life will be improved. Both sildenafil and tadalafil have been approved by the FDA for pulmonary arterial hypertension treatment.
In recent years, studies have shown that PDE5 inhibitors also have potential in other fields. For example, they can treat heart failure and coronary artery disease. In addition, they may also have anti-inflammatory and anti-fibrotic effects, so they are gradually being studied for the treatment of more diseases. Due to PDE5 inhibitors’ ability to induce vasodilation, they may be used to treat some diseases related to blood flow restriction in the future.
Side Effects and Risks
Although PDE5 inhibitors have significant therapeutic effects, they are not completely free from side effects and risks.
PDE5 inhibitors’ common side effects include headaches, indigestion, and nasal congestion. These symptoms are usually caused by drug-induced vasodilation.
After taking PDE5 inhibitors, some patients may experience temporary blurred vision or abnormal color perception of blue-green. Although this side effect is relatively rare, it deserves attention because the PDE6 enzyme is also present in the retina, and certain PDE5 inhibitors have a certain inhibitory effect on PDE6.
When they are used in combination with other antihypertensive drugs such as nitrates, PDE5 inhibitors may cause hypotension. This interaction may lead to severe blood pressure drops and even endanger life. Therefore, patients who are currently using nitrate drugs, are prohibited from using PDE5 inhibitors.
