ECHEMI 2019-04-16

Xi'an Medical Insurance Bureau issued an emergency notice that a batch of drugs can replace 4 + 7 varieties. On April 1, Xi'an Medical Insurance Bureau issued the Emergency Notice on Implementing the Monitoring Task of the State 4+7 Pilot Work by the Office of the Leading Group for Implementing the Centralized Procurement and Use of Drugs of the State Organization in Xi'an (hereinafter referred to as the Notice). According to the Notice, drug manufacturers, drug circulation enterprises, drug procurement (use), drug use, drug cost structure and drug quality should report to the relevant departments.

(1) Each manufacturer of 25 selected drugs submits data directly to Shanghai Joint Purchase.

Production enterprises need to submit data including inventory turnover days, stop and resume production, price of core raw materials, price of core accessories, etc. From the above data requirements, we can see that the state pays close attention to the supply and price changes of selected drugs.

(2) Each circulation enterprise responsible for the distribution of 25 selected drugs submits data directly to the Municipal Medical Insurance Bureau. Circulation enterprises need to focus on reporting the situation of return and channel inventory.

Circulation enterprises need to focus on reporting the situation of return and channel inventory. At present, in addition to clear time and percentage requirements for repayment, the situation of repayment is also included in the scope of monitoring, and the guarantee of repayment should be more vigorous.
(3) According to the requirements of the State Notification, the Shaanxi Pharmaceutical Equipment Centralized Purchasing Platform directly exchanges data from Xi'an to Shanghai Joint Purchasing Data Exchange Platform. For the need of data aggregation, relevant data should be fed back to.
The main information that needs feedback is purchase volume, purchase amount, cumulative purchase amount and cumulative purchase amount.
(4) The Municipal Health key committee took the lead in supervising and guiding all public medical institutions in Xi'an to report data on two indicators of "ensuring quality and cost control", and copied them to the municipal health insurance bureau. After the Municipal Health key committee has delimited the scope of alternative drugs, it shall submit them to the pilot office for examination. For the use of drugs, it is necessary to count the number of monthly prescriptions of selected drugs, the number of monthly prescriptions of original drugs with the same generic name, the number of drug exchanges (original research drugs changed into selected drugs), the number of second drug exchanges (original research drugs changed back to selected drugs, then returned to original research drugs) and other indicators. From the above indicators, it can be seen that the relevant departments should monitor the use of selected drugs and original research drugs in order to observe the strength of substituting selected drugs for original research drugs.
In addition, the number of drug change and the number of second drug change further reflect the substitution of selected drugs, and whether the selected drugs encounter obstacles at the patient end.

If there are more people who change drugs twice, it may indicate that many patients feel worse than the original drug after using the selected drug, and have to change back to the original drug. If this data is relatively high, the replacement work of the selected drug may need further adjustment. In addition to drug use, drug cost structure and drug quality also need to be monitored continuously. The situation of drug quality mainly focuses on adverse drug reactions, substandard drug quality and public complaints. Finally, the Xi'an Medical Insurance Bureau also provides a reference range for similar alternative varieties of selected medicines. It can be seen that once the supply of selected medicines has problems, they can be basically replaced, and under some specific conditions alternative medicines or usher in opportunities.
