Function and Efficacy

Tetracycline antibiotics. This product is a broad-spectrum antibacterial agent with bactericidal effects at high concentrations. Many Rickettsia, Mycoplasma, Chlamydia, atypical mycobacteria, and spirochetes are also sensitive to this product. This product is more effective against Gram-positive bacteria than Gram-negative bacteria, but Enterococcus is resistant to it. Others such as Actinomycetes, Bacillus anthracis, Listeria monocytogenes, Clostridium, Nocardia, Vibrio, Brucella, Campylobacter, and Yersinia are sensitive to this product. This product has certain antibacterial activity against Neisseria gonorrhoeae, but penicillin-resistant Neisseria gonorrhoeae is also resistant to doxycycline. Due to the widespread use of tetracyclines over the years, common clinical pathogens have become seriously resistant to this product, including Gram-positive bacteria such as Staphylococcus and most Gram-negative bacilli. There is cross-resistance between this product and different varieties of tetracycline antibiotics. The mechanism of action of this product is that the drug can specifically bind to the A position of the bacterial ribosome 30S subunit, inhibiting the growth of the peptide chain and affecting the synthesis of bacterial protein. Animal experiments have confirmed that this product is teratogenic. In vitro tests have shown that this product may be mutagenic at a certain concentration.

Indication

1. This product can be used for the following diseases: (1) Rickettsial diseases, such as epidemic typhus, endemic typhus, Rocky Mountain fever, scrub typhus and Q fever. (2) Mycoplasma infections. (3) Chlamydia infections, including psittacosis, venereal diseases, lymphoedema, nonspecific urethritis, salpingitis, cervicitis and trachoma. (4) Relapsing fever. (5) Brucellosis. (6) Cholera. (7) Tularemia. (8) Plague. (9) Chancroid. It should be used in combination with aminoglycosides to treat brucellosis and plague. 2. Due to the serious resistance of common pathogens to tetracyclines, this product is only indicated when the pathogens are sensitive to it. Most Staphylococci are resistant to this product. 3. This product can be used for tetanus, gas gangrene, yaws, syphilis, gonorrhea, leptospirosis, and actinomycetes and listeria infections in patients who are allergic to penicillins. 4. It can be used as an adjuvant treatment for patients with moderate to severe acne.

Usage and Dosage

The dosage of the drug cannot be increased or decreased at will. The specific dosage should be referred to the instructions or prescribed by the doctor. Specifically, for doxycycline hydrochloride tablets, the dosage for different diseases is also different. Antibacterial and antiparasitic infections: Adults, 100 mg on the first day, once every 12 hours, followed by 100-200 mg, once a day, or 50-100 mg, once every 12 hours. Gonorrheal Neisseria urethritis and cervicitis: 100 mg once, once every 12 hours. A total of 7 days. Non-gonococcal Neisseria urethritis, caused by Chlamydia trachomatis or Ureaplasma urealyticum, and simple urethritis, cervicitis or rectal infection caused by Chlamydia trachomatis: 100 mg once, twice a day, for a course of at least 7 days. Syphilis: 150 mg once, once every 12 hours, for a course of at least 10 days. Children over 8 years old should take 2.2 mg/kg of body weight once every 12 hours on the first day, followed by 2.2 to 4.4 mg/kg of body weight once a day, or 2.2 mg/kg of body weight once every 12 hours. Children weighing more than 45 kg should take the same dosage as adults.

Adverse Reactions

1. Digestive system: Oral administration of this product may cause gastrointestinal reactions such as nausea, vomiting, abdominal pain, and diarrhea. There are occasional reports of esophagitis and esophageal ulcers, which mostly occur in patients who are bedridden immediately after taking the drug. 2. Hepatotoxicity: It is prone to occur in patients with fatty liver degeneration and pregnant women, and it may also occur in patients without the above conditions. Pancreatitis may occur occasionally, and pancreatitis caused by this product may also occur simultaneously with hepatotoxicity, and the patient does not have primary liver disease. 3. Allergic reactions: Mostly maculopapular rash and erythema, a few patients may have urticaria, angioneurotic edema, allergic purpura, pericarditis, and aggravation of skin lesions in systemic lupus erythematosus, and exfoliative dermatitis is not common. Occasionally, anaphylactic shock and asthma occur. Some patients who use this product may have photosensitivity when exposed to the sun. Therefore, it is recommended that patients should not be directly exposed to sunlight or ultraviolet rays while taking this product, and the drug should be stopped immediately if erythema appears on the skin. 4. Blood system: Occasionally it may cause hemolytic anemia, thrombocytopenia, neutropenia and eosinophilia. 5. Central nervous system: Occasionally it may cause benign intracranial hypertension, which may manifest as headache, vomiting, papilledema, etc., which may be relieved after stopping the drug. 6. Superinfection: Long-term use of this product may cause digestive tract, respiratory tract and urinary tract infections caused by drug-resistant Staphylococcus aureus, Gram-negative bacteria and fungi, and in severe cases may cause sepsis. 7. The use of tetracyclines can reduce the normal flora in the human body, and cause vitamin deficiency, fungal growth, dry mouth, pharyngitis, angular cheilitis and glossitis.

Precautions

Patients with a history of allergy to tetracyclines are contraindicated.

Special Population Medication

Precautions for children: Children under 8 years old are prohibited to use. Precautions for pregnancy and lactation: 1. This product can enter the fetus through the placental barrier and deposit in the calcium area of teeth and bones, causing fetal tooth discoloration, poor enamel regeneration and inhibiting fetal bone growth. This type of drug has teratogenic effects in animal experiments, so pregnant women should not use it. 2. This product can be secreted from breast milk, and the concentration in breast milk is relatively high. Lactating women should stop breastfeeding when using it. Precautions for the elderly:

Drug Interactions

1. This product can inhibit the activity of plasma prothrombin, so patients receiving anticoagulant therapy need to adjust the dose of anticoagulants. 2. When barbiturates, phenytoin or carbamazepine are used with this product, the above drugs can induce the activity of microsomal enzymes and cause a decrease in the blood concentration of doxycycline, so the dose of doxycycline must be adjusted.
