Function and Efficacy
Potassium is the main cation in cells, with a concentration of 150-160 mmol/L, while the main cation outside cells is sodium ions, and the serum potassium concentration is only 3.5-5.0 mmol/L. The body mainly relies on Na+-K+ ATPase on the cell membrane to maintain the difference in K+ and Na+ concentrations inside and outside cells. The acid-base balance in the body has an impact on potassium metabolism. For example, when acidosis occurs, H+ enters the cell. In order to maintain the potential difference inside and outside the cell, K+ is released outside the cell, causing or aggravating hyperkalemia. Metabolic disorders can also affect acid-base balance. Indication
Electrolyte supplement. Used for: 1. Treatment of hypokalemia caused by various reasons, such as insufficient food intake, vomiting, severe diarrhea, use of potassium-excreting diuretics, hypokalemic familial periodic paralysis, long-term use of glucocorticoids and hypokalemia caused by supplementation of hypertonic glucose, etc. 2. Prevention of hypokalemia. When patients have potassium loss, especially if hypokalemia is harmful to patients (such as patients using digitalis drugs), preventive potassium salt supplementation is required, such as eating very little, severe or chronic diarrhea, long-term use of adrenocortical hormones, potassium-losing nephropathy, Bartter syndrome, etc. 3. Frequent, multifocal premature beats or rapid arrhythmias caused by digitalis poisoning.
Usage and Dosage
The potassium content of each 1g of potassium chloride is 13.4mmol. It is used for severe hypokalemia or those who cannot take it orally. The general usage is to add 10-15ml of 10% potassium chloride injection to 500ml of 5% glucose injection and drip. The dose, concentration and speed of potassium supplementation are determined according to the clinical condition, blood potassium concentration and improvement of the potassium deficiency pattern on the electrocardiogram. The potassium concentration shall not exceed 3.4g/L (45mmol/L), the potassium supplementation rate shall not exceed 0.75g/hour (10mmol/hour), and the daily potassium supplementation amount shall be 3-4.5g (40-60mmol). Adverse Reactions
1. When the intravenous drip concentration is high, the drip speed is fast or the vein is thin, it is easy to stimulate the venous endothelium and cause pain, or even phlebitis. 2. Hyperkalemia. It is easy to occur when the drug is overdosed, the drip speed is fast or the original renal function is damaged. It manifests as weakness, fatigue, numbness of the hands, feet, lips and mouth, unexplained anxiety, confusion, dyspnea, slow heart rate, arrhythmia, conduction block, and even cardiac arrest. The electrocardiogram shows high and sharp T waves, and the P-R interval is gradually prolonged. The P wave disappears, the QRS wave widens, and a sine wave appears.
Precautions
1. It is contraindicated for patients with hyperkalemia. 2. It is contraindicated for patients with acute renal insufficiency or chronic renal insufficiency.
Special Population Medication
Precautions for children: Precautions for pregnancy and lactation: No special findings. Precautions for the elderly: The kidney's K+ clearance function of the elderly is reduced, and hyperkalemia is more likely to occur when potassium salts are used.
Drug Interactions
1. Adrenal glucocorticoids, especially those with more obvious mineralocorticoid effects, adrenal mineralocorticoids and adrenocorticotropic hormone (ACTH), can promote urinary potassium excretion, and when used in combination with this product, the efficacy of potassium salts will be reduced. 2. Anticholinergic drugs can aggravate the gastrointestinal irritation of oral potassium salts, especially potassium chloride. 3. Non-steroidal anti-inflammatory analgesics aggravate the gastrointestinal reactions of oral potassium salts. 4. When used in combination with stored blood (containing 30mmol/L potassium for less than 10 days of storage, 65mmol/L potassium for more than 10 days of storage), potassium-containing drugs and potassium-sparing diuretics, the chance of hyperkalemia increases, especially in patients with renal damage. 5.
