Function and Efficacy

This product belongs to the lincomycin antibiotics, which is a derivative of lincomycin. Its antibacterial spectrum is the same as that of lincomycin, and its antibacterial activity is 4 to 8 times stronger than that of lincomycin. It has high antibacterial activity against Gram-positive bacteria such as Staphylococcus (including penicillin-resistant strains), Streptococcus, Corynebacterium diphtheriae, Bacillus anthracis, etc. It also has good antibacterial activity against Gram-negative anaerobic bacteria. Most of the Bacteroides, including Bacteroides fragilis, Fusobacterium, Peptococcus, Peptostreptococcus, and Clostridium perfringens, are highly sensitive to this product. Gram-negative aerobic bacteria, including Haemophilus influenzae, Neisseria, and Mycoplasma, are resistant to this product. There is no cross-resistance between this product and penicillin, chloramphenicol, cephalosporins and tetracyclines, but it has partial cross-resistance with macrolides and complete cross-resistance with lincomycin. The mechanism of action of this product is to bind to the 50S subunit of the bacterial ribosome, prevent the extension of the peptide chain, and thus inhibit the protein synthesis of bacterial cells. This product is an antibacterial drug, but at high concentrations, it also has a bactericidal effect on certain bacteria.

Appearance

This product is a colorless or almost colorless clear liquid.

Indication

This product is suitable for moderate and severe infections caused by Streptococcus, Staphylococcus and anaerobic bacteria (including Bacteroides fragilis, Clostridium perfringens, Actinomycetes, etc.), such as aspiration pneumonia, empyema, lung abscess, osteomyelitis, abdominal infection, pelvic infection and sepsis.

Usage and Dosage

Intravenous drip. Adults: 0.6-1.2g per day, divided into 2-4 times; severe infection: 1.2-2.4g per day, divided into 2-4 times. Children 4 weeks and older: 15-25mg/kg per day, divided into 3-4 times; severe infection: 25-40mg/kg per day, divided into 3-4 times.

Adverse Reactions

1. Gastrointestinal reactions: common symptoms include nausea, vomiting, abdominal pain, diarrhea, etc.; severe cases include abdominal colic, abdominal tenderness, severe diarrhea (watery or purulent), accompanied by fever, abnormal thirst and fatigue. Diarrhea, enteritis and pseudomembranous enteritis may occur in the early stage of medication or several weeks after discontinuation of medication. 2. Blood system: occasionally leukopenia, neutropenia, eosinophilia and thrombocytopenia may occur; rare aplastic anemia. 3. Allergic reactions: rash, itching, etc. may be seen, occasionally urticaria, angioedema and serum sickness reaction, etc., rare exfoliative dermatitis, bullous dermatitis, erythema multiforme and Steven-Johnson syndrome. 4. Abnormal liver and kidney function, such as elevated serum aminotransferase, jaundice, etc. 5. Intravenous infusion may cause phlebitis; intramuscular injection may cause local pain, nodules and sterile abscesses. 6. Others: tinnitus, dizziness, candidal infection, etc.

Precautions

Patients who are allergic to this product and lincomycins are contraindicated.

Special Population Medication

Precautions for children: This product is contraindicated for infants under 4 weeks of age. Other children should pay attention to the function of important organs when taking this product. Precautions for pregnancy and lactation: 1. Animal experiments show that this product has no effect on the fetus, but there is still a lack of experience in its application to pregnant women, and this product can pass through the placenta, so pregnant women should use it with caution. 2. This product can be secreted into breast milk, so lactating women should use it with caution, and breastfeeding should be suspended when using this product. Precautions for the elderly: Elderly people with serious underlying diseases are prone to adverse reactions such as diarrhea or pseudomembranous colitis, and need to be closely observed during medication.

Drug Interactions

1. This product can enhance the neuromuscular blockade of inhaled anesthetics, leading to skeletal muscle weakness and respiratory depression or paralysis (apnea). Caution should be taken when using it during or after surgery. Treatment with anticholinesterase drugs or calcium salts is expected to be effective. 2. This product can cause pseudomembranous colitis with severe watery diarrhea during or even a few weeks after the course of treatment when used in combination with antiperistaltic antidiarrheal drugs and antidiarrheal drugs containing white clay. Because it can delay the excretion of colonic endotoxins, thereby causing prolonged and aggravated diarrhea, this product should not be used in combination with antiperistaltic antidiarrheal drugs. When used in combination with antidiarrheal drugs containing white clay, the absorption of this product will be significantly reduced, so the two should not be taken at the same time, and a certain interval (at least 2 hours) is required. 3. This product has a neuromuscular blocking effect and can enhance the effect of neuromuscular blocking drugs. The two should be avoided in combination. When this product is used in combination with anti-myasthenic drugs, the latter will weaken the effect on skeletal muscles. In order to control the symptoms of myasthenia gravis, the dose of anti-myasthenic drugs should be adjusted when used in combination. 4. Chloramphenicol or erythromycin can replace this product at the target site, or inhibit the binding of this product to the bacterial ribosome 50S subunit. In vitro tests show that this product has an antagonistic effect with erythromycin, so this product should not be used in combination with chloramphenicol or erythromycin. 5. When used in combination with opioid analgesics, the respiratory depression of this product and the central respiratory depression of opioids may add up to prolong respiratory depression or cause respiratory paralysis (apnea), so patients must be closely observed or monitored. 6. This product should not be added to infusions with complex components to avoid incompatibility.
