Function and Efficacy
This product is a formyl derivative of tetrahydrofolate. It is mainly used to rescue high-dose methotrexate and other folic acid antagonists. The main function of methotrexate is to bind to dihydrofolate reductase, blocking the conversion of dihydrofolate to tetrahydrofolate, thereby inhibiting DNA synthesis. After entering the human body, this product is converted into tetrahydrofolate by tetrahydrofolate reductase, which can effectively counteract the toxic reaction caused by methotrexate. This product can limit the degree of damage to normal cells caused by methotrexate and reverse the reaction of methotrexate to bone marrow and gastrointestinal mucosa, but it has no obvious effect on the existing methotrexate neurotoxicity.

Indication
1. Used for follow-up treatment of high-dose methotrexate to reduce toxicity (calcium folinate rescue). Also used to treat patients with negligent methotrexate overdose and impaired methotrexate excretion. Combined with fluorouracil, it can improve the efficacy of fluorouracil and is commonly used in the treatment of colorectal cancer and gastric cancer. 2. Also used for megaloblastic anemia caused by sprue, malnutrition, pregnancy or infancy. It is not suitable for vitamin B12 deficiency anemia when oral folic acid is ineffective.
Usage and Dosage
1 As a "rescue" therapy for methotrexate, the commonly used dose is 5-15 mg, taken orally every 6-8 hours for 2 consecutive days. The blood concentration of methotrexate is controlled below 5×10-8 mol/L according to the results of blood concentration determination. 2 As a detoxification of pyrimethamine or trimethoprim, the daily oral dose is 5-15 mg, depending on the poisoning situation. 3 For megaloblastic anemia, take 15 mg orally daily. 4 When used in combination with fluorouracil, take 20-30 mg/m2 of body surface area orally, half an hour after taking fluorouracil.
Adverse Reactions
Rarely, allergic reactions such as rash, hives, or asthma may occur occasionally.
Precautions
It is contraindicated in pernicious anemia or megaloblastic anemia caused by vitamin B12 deficiency.
Drug Interactions
The use of this product in large doses together with barbiturates, primidone or phenytoin sodium may affect the anti-epileptic effect.
