Function and Efficacy
Pharmacological action: This product contains B vitamins necessary for normal human metabolism. Thiamine nitrate forms thiamine pyrophosphate in the body, which is a coenzyme necessary for carbohydrate metabolism and can maintain the normal functions of the heart, nerves and digestive system. Thiamine nitrate can inhibit the activity of cholinesterase. When it is deficient, the activity of cholinesterase is enhanced, and the hydrolysis of acetylcholine is accelerated, leading to nerve impulse conduction disorders and affecting gastrointestinal and myocardial functions. Pyridoxine hydrochloride is converted into pyridoxal phosphate in red blood cells, and acts as a coenzyme for various metabolic functions of proteins, carbohydrates and lipids. Pyridoxine hydrochloride also participates in the conversion of tryptophan into niacin or 5-hydroxytryptamine. Vitamin B12 participates in methyl conversion and folic acid metabolism in the body, and promotes the conversion of 5-methyltetrahydrofolate into tetrahydrofolate. When it is deficient, it leads to DNA synthesis disorders and affects the maturation of red blood cells. Vitamin B12 promotes the conversion of methylmalonic acid into succinic acid and participates in the tricarboxylic acid cycle. This effect is related to the synthesis of nerve spinal sphingolipids and the maintenance of the integrity of spinal nerve fiber function. The nerve damage caused by vitamin B12 deficiency may be related to this. Toxicological study: Acute toxicity test shows that the LD50 value of this product is 2.15812g/kg, and the 95% confidence limit is 2.04786-2.27432g/kg. Safety test shows that this product has no obvious irritation to the quadriceps muscle of rabbits; this product has no obvious irritation to the venous blood vessels of rabbits. Allergy test shows that this product has no sensitization effect on guinea pigs: In vitro hemolysis test shows that this product has no hemolysis or agglutination effect on rabbit red blood cells at all concentrations within 4 hours.
Ingredients
This product is a compound preparation, and its components are: thiamine nitrate, pyridoxine hydrochloride, vitamin B12. Each bottle contains 2.0 mg of thiamine nitrate, 30 mg of pyridoxine hydrochloride, and 2.5 μg of vitamin B12; the excipients are: glycine, dextran 40, and disodium edetate.

Appearance
This product is white, light red or red loose lumps.
Indication
It is used for peripheral nerve injury, multiple neuritis, trigeminal neuralgia, sciatica; prevention and treatment of isoniazid poisoning, pregnancy, radiation sickness, vomiting caused by anti-tumor drugs, seborrheic dermatitis, pernicious anemia, nutritional anemia, etc. It can also be used as a nutritional supplement for patients with B vitamin intake disorders.
Usage and Dosage
For intramuscular injection, adults take 1 vial each time, once a day; children take the medicine according to the doctor's advice. For intravenous drip, adults take 1-2 vials each time, once a day. Dissolve it with 10 ml of 5% or 10% glucose injection or 10 ml of sterile water for injection before use, and add 100-250 ml of 5% or 10% glucose injection after dissolution and infuse it intravenously.
Adverse Reactions
Occasionally, dizziness, fatigue, nausea, vomiting, rash, itching, and hard lumps may occur locally after intramuscular injection, which may gradually disappear after hot compress or drug withdrawal. Very few may have allergic reactions such as allergic asthma.

Special Population Medication
Precautions for children: Children should use a reduced dosage as appropriate. Precautions for pregnancy and lactation: Pregnant women who receive large amounts of pyridoxine hydrochloride may cause pyridoxine hydrochloride dependence syndrome in newborns. Normal intake of required amounts by lactating women has no adverse effects on infants. Precautions for the elderly: In clinical applications, no significant differences in efficacy and safety were found between elderly patients and the general population when using the recommended dose of this product.
Drug Interactions
Chloramphenicol, cycloserine, ethionamide, hydralazine hydrochloride, immunosuppressants including adrenocortical hormones, cyclophosphamide, cyclosporine, isoniazid, penicillamine and other drugs can antagonize pyridoxine hydrochloride or increase the renal excretion of pyridoxine hydrochloride, which can cause anemia or peripheral neuritis. The combination of levodopa and a small dose of pyridoxine hydrochloride (5 mg per day) can antagonize the anti-tremor effect of levodopa. But it has no effect on carbidopa. This product is easily decomposed in alkaline solution, and it is easy to cause deterioration when combined with alkaline drugs such as sodium bicarbonate and sodium citrate.
