Function and Efficacy
This product belongs to the lincomycin antibiotics. It is a derivative of lincomycin, with the same antibacterial spectrum as lincomycin, and its antibacterial activity is 4 to 8 times stronger than that of lincomycin. It has high antibacterial activity against Gram-positive bacteria such as Staphylococcus (including penicillin-resistant strains), Streptococcus, Corynebacterium diphtheriae, Bacillus anthracis, etc. It also has good antibacterial activity against Gram-negative anaerobic bacteria. Most of the Bacteroides, including Bacteroides fragilis, Fusobacterium, Peptococcus, Peptostreptococcus, and Clostridium perfringens, are highly sensitive to this product. Gram Negative aerobic bacteria including Haemophilus influenzae, Neisseria and Mycoplasma are resistant to this product. There is no cross-resistance between this product and penicillin, chloramphenicol, cephalosporins and tetracyclines, partial cross-resistance with macrolides, and complete cross-resistance with lincomycin. The mechanism of action of this product is to bind to the 50S subunit of the bacterial ribosome, prevent the extension of the peptide chain, and thus inhibit the protein synthesis of bacterial cells. This product is an antibacterial drug, but at high concentrations, it also has a bactericidal effect on certain bacteria.

Indication
The following infectious diseases caused by Gram-positive bacteria: 1. Tonsils, suppurative otitis media, sinusitis, etc. 2. Acute bronchitis, acute exacerbation of chronic bronchitis, pneumonia, lung abscess and bronchiectasis combined infection, etc. 3. Skin and soft tissue infections: furuncle, carbuncle, abscess, cellulitis, trauma, burns and surgical infection, etc. 4. Urinary tract infection: acute urethritis, acute pyelonephritis, prostatitis, etc. 5. Others: myelitis, sepsis, peritonitis and oral infection, etc. Various infectious diseases caused by anaerobic bacteria: 1. Empyema, muscle abscess, anaerobic pneumonia. 2. Skin and soft tissue infection, sepsis. 3. Intra-abdominal infection: peritonitis, intra-abdominal abscess. 4. Female pelvic and genital infections: endometritis, non-gonococcal fallopian tube and ovarian abscess, pelvic cellulitis and post-gynecological surgery infection, etc.
Usage and Dosage
Adults: deep intramuscular injection or intravenous drip. Moderate infection: 0.6-1.2g/day, divided into 2-3 times. Severe infection: 1.2-2.7g/day, divided into 2-3 times. Or as directed by a doctor. Children: intramuscular injection or intravenous drip. Moderate infection: 15-25mg/kg/day, divided into 2-3 times. Severe infection: 25-40mg/kg/day, divided into 2-3 times. Or as directed by a doctor. Intravenous drip: dilute 0.3g of this product with 100ml of normal saline or 5% glucose solution and drip intravenously for 30 minutes.
Adverse Reactions
1. Local reaction: after intramuscular injection, slight pain may occasionally occur at the injection site. Long-term intravenous infusion should pay attention to the occurrence of phlebitis. 2. Gastrointestinal reaction: nausea, vomiting, abdominal pain and diarrhea may occur occasionally. 3. Allergic reaction: a few patients may experience drug-induced rash and dermatitis, and a very small number of patients may experience anaphylactic shock. 4. There is basically no toxic reaction to the hematopoietic system. It may occasionally cause neutropenia, eosinophilia, thrombocytopenia, etc., which are generally mild and transient. 5. A few patients may experience transient alkaline phosphatase, mild elevation of serum transaminase and jaundice. 6. A very small number of patients may experience hematuria, which is mostly related to excessive dosage and too fast administration speed. It can generally recover after treatment. 7. A very small number of patients may develop pseudomembranous enterocolitis.
Precautions
This product has cross-resistance with lincomycin and is contraindicated in patients with a history of allergy to this product or lincomycin.
Special Population Medication
Precautions for children: The efficacy and safety of the drug for children under 12 years old have not been established. Precautions for pregnancy and lactation: The efficacy and safety of the drug for pregnant women have not been established, and this product can pass through the placenta, so pregnant women should use it with caution; this product can be secreted into breast milk, so lactating women should use it with caution. If this product must be used, breastfeeding should be suspended. Precautions for the elderly: The efficacy and safety of the drug for elderly patients over 65 years old have not been established. Elderly people with serious underlying diseases are prone to adverse reactions such as diarrhea or pseudomembranous colitis, and need to be closely observed when taking the drug.
Drug Interactions
1. This product can enhance the neuromuscular blockade of inhaled anesthetics, leading to skeletal muscle weakness and respiratory depression or paralysis (apnea). Caution should be taken when used in combination during or after surgery. Treatment with anticholinesterase drugs or calcium salts is expected to be effective. 2. This product can cause pseudomembranous colitis with severe watery diarrhea during or even a few weeks after the course of treatment when used in combination with antiperistaltic antidiarrheal drugs and antidiarrheal drugs containing white clay. Because it can delay the excretion of colonic endotoxins, thereby causing prolonged and aggravated diarrhea, this product should not be used in combination with antiperistaltic antidiarrheal drugs. When used in combination with antidiarrheal drugs containing white clay, the absorption of this product will be significantly reduced. Therefore, the two should not be taken at the same time and a certain interval (at least 2 hours) is required. 3. This product has a neuromuscular blocking effect and can enhance the effect of neuromuscular blocking drugs. The two should be avoided in combination. When this product is used in combination with antimyasthenic drugs, the latter will weaken the effect on skeletal muscles. In order to control the symptoms of myasthenia gravis, the dose of antimyasthenic drugs should be adjusted when used in combination. 4. Chloramphenicol or erythromycin can replace this product at the target site, or inhibit the binding of this product to the bacterial ribosome 50S subunit. In vitro tests show that this product has an antagonistic effect with erythromycin, so this product should not be used in combination with chloramphenicol or erythromycin. 5. When used in combination with opioid analgesics, the respiratory depression of this product and the central respiratory depression of opioids may add up to prolong respiratory depression or cause respiratory paralysis (apnea), so patients must be closely observed or monitored. 6. This product should not be added to infusions with complex components to avoid incompatibility.
