Function and Efficacy
Elemene emulsion is an effective anticancer ingredient extracted from the ginger plant Curcuma zedoaria. Its main biological activity is to reduce the mitotic ability of tumor cells, induce tumor cell apoptosis, and inhibit the growth of tumor cells. Pharmacological experiments have shown that intraperitoneal injection of elemene emulsion has a significant inhibitory effect on the synthesis of DNA, RNA and protein in tumor cells. The drug can also directly act on the cell membrane to rupture tumor cells, change and enhance the immunogenicity of tumor cells, and induce and promote the body's immune response to tumor cells. This product has less toxic and side effects and has little effect on normal cells and surrounding leukocytes. The intravenous injection LD50 is (270.07±18.93) mg/kg, and the oral LD50 is greater than 5g/kg. The commonly used dose has no teratogenic or mutagenic effects on mice.

Appearance
This product is a milky white uniform emulsion.
Indication
Clinically, it is mostly used as the first-line treatment for malignant chest and peritoneal effusions, brain tumors, respiratory and digestive tract tumors, and as the second-line treatment for gynecological tumors, breast cancer, skin cancer, bone metastasis, lymphoma, leukemia, etc.
Usage and Dosage
1. Thoracic injection: After draining as much pleural effusion as possible with a trocar (closed) drainage, first inject 10ml of 2% procaine or lidocaine injection or an appropriate amount to control pain, and then inject into the thoracic cavity at a dose of 200-300mg/m2 of body surface area. After injection, ask the patient to change position several times to increase the contact area of the drug solution, 1-2 times/week, 2 weeks as a course of treatment. 2. Intraperitoneal injection: Drain as much as possible of the ascites, first inject 5-10ml of 2% procaine or lidocaine injection or an appropriate amount and 5-10mg dexamethasone to control pain, then 300-400mg/m2, dilute with 250ml of physiological saline, and slowly drip into the abdominal cavity. The drip speed depends on the patient's tolerance. After injection, the patient should change position, use the drug 1-2 times/week, 2 weeks as a course of treatment. 3 Intravenous injection: once a day, 400-600 mg/time, 15 days as a course of treatment. It is best to use a subclavian deep vein cannula for administration. If conditions are unavailable, a thicker vein should be selected and used alternately in both arms. It is best to use a cannula. First, use 250 ml of normal saline to open the venous access. To prevent the occurrence of phlebitis, 5-10 ml of dexamethasone can be added on the 1st to 5th day, flushed in with a small pot, and then the product is diluted in 300-400 ml of normal saline to flush the blood vessels. 4 Oral administration: 100-200 mg/time, 3 times/day. Take half an hour before meals. Esophageal cancer and gastric cancer should be swallowed in small sips to facilitate the contact of the drug solution with the affected area. 5 Interventional administration: Literature reports that elemene emulsion is mainly used for interventional treatment of liver cancer, lung cancer, head and neck tumors, gastric cancer, and esophageal cancer. Treatment methods include: perfusion, embolization, and PCS implantation. Due to the different locations and methods of target blood vessels, the dosage is 400-100 ml/time, 1-15 times/month. If the interventional dosage is less than 4 times/month, it is recommended to administer 200-500 mg/day intravenously at the same time. 6 Bladder instillation: used for neoadjuvant chemotherapy before and after bladder cancer surgery. Starting from the tenth day after surgery, 300 mg of elemene emulsion is injected through the catheter, and 10 ml of 2% lidocaine is added for bladder instillation. The body position is turned over every 30 minutes (supine, lying, left and right side positions) until urination after two hours, twice a week, for 6 consecutive weeks. Then once a month for up to one year to prevent recurrence and metastasis. For supportive treatment of advanced bladder cancer, refer to the preoperative medication method. 7 Rectal instillation: Use an anal tube or catheter to instill 200 mg, 1-2 times a day, retain the enema for 2 hours, and a course of 10 days. 8 Local tumor injection: First use lidocaine or other local anesthetics to inject multiple points on the tumor (note that the infiltration surface of the local anesthetic should be complete). After 3-5 minutes, inject this product at 50-75 mg/time at multiple points depending on the size of the tumor. The dosage and frequency of use will be determined by the attending physician according to different situations.
Adverse Reactions
Some patients may experience phlebitis, fever, local pain, allergic reactions, and mild gastrointestinal reactions after taking the medicine.
Precautions
Use with caution in patients with high fever or pleural effusion and ascites complicated with infection.
Drug Interactions
It has a synergistic effect when used in combination with radiotherapy or other chemotherapy drugs and biological response modifiers, and it has a synergistic effect when used in combination with warming therapy.
