Function and Efficacy

This product is a compound antibiotic composed of amoxicillin and flucloxacillin sodium in a ratio of 1:1. Amoxicillin is a semi-synthetic broad-spectrum penicillin belonging to the aminopenicillin class. It has bactericidal effects on both Gram-negative and Gram-positive bacteria, and its characteristics are broad-spectrum and not resistant to penicillinase. Flucloxacillin is a semi-synthetic isoxazole penicillin, which is not easily destroyed by penicillinase and has a bactericidal effect on penicillinase-resistant Staphylococcus aureus. It is mainly used for penicillin-resistant Staphylococcus aureus infections, but Gram-negative bacteria are resistant to flucloxacillin. The antibacterial mechanism of both is the same as that of penicillin, and both play an antibacterial role by binding to bacterial penicillin-binding proteins (PBPs) and interfering with the synthesis of bacterial cell walls. When combined with amoxicillin and flucloxacillin sodium, they can play an antibacterial role against enzyme-producing strains of Staphylococcus aureus and sensitive strains of certain Gram-negative bacteria.

Appearance

This product is in capsule form, and the content is white or off-white powder, which is hygroscopic.

Indication

This product is suitable for respiratory tract infections, digestive system infections, urinary tract infections, skin and soft tissue infections, bone and joint infections, oral and ear, nose and throat infections, etc. caused by sensitive bacteria.

Usage and Dosage

Oral administration. Adults: 500 mg (2 tablets) 3 times a day. Children aged 2 to 12 years: 250 mg (1 tablet) 3 times a day. The dosage can be increased appropriately for severe patients. To ensure maximum absorption of the drug, the drug should be administered on an empty stomach, usually 1 hour before a meal.

Adverse Reactions

1. Allergic reactions: more common, including rash, drug fever, asthma, etc., and may occasionally cause anaphylactic shock. 2. Gastrointestinal reactions: including nausea, vomiting, diarrhea and other reactions, and occasionally pseudomembranous colitis. 3. Hepatotoxic type: a small number of patients experience increased ALT and AST after taking the drug, and there are also reports of acute hepatic cholestasis. 4. Nephrotoxic type: there are occasional reports of acute interstitial nephritis. 5. Nervous system reactions: large doses of intravenous flucloxacillin can cause nervous system reactions such as headache, convulsions, and seizures. This reaction is particularly common in patients with impaired renal function. There are occasional reports of excitement, anxiety or insomnia. 6. Blood system reactions: there are occasional reports of neutropenia, eosinophilia, thrombocytopenia or hemolytic anemia. 7. Long-term, high-dose use of the drug may cause bacteria

Precautions

1. Patients who are allergic to any component of this drug or other penicillins. 2. Patients with infectious mononucleosis, cytomegalovirus infection, lymphocytic leukemia, lymphoma, etc.

Special Population Medication

Precautions for children: Both flucloxacillin and amoxicillin have clinical experience in children and newborns. The dosage can be adjusted to 1/4 to 1/2 of the adult dosage, and the liver and kidney function of children should be monitored during medication (see Dosage and Administration). Precautions for pregnancy and lactation: Both flucloxacillin and amoxicillin can pass through the placental barrier and be secreted through breast milk, so the use of this product by pregnant and lactating women may cause allergic reactions, diarrhea and candidiasis in infants. The safety of flucloxacillin for women in the first trimester of pregnancy has not yet been established, and the use of the drug in the second and third trimesters of pregnancy and during lactation has no effect on newborns; clinical control studies of amoxicillin have shown that the use of the drug during pregnancy has no significant effect on the fetus. Animal experiments have shown that the use of flucloxacillin and amoxicillin during pregnancy does not harm the fetus. Although the limited clinical research data and animal studies have shown that this product is safe for use during pregnancy and lactation, considering the potential toxicity of penicillins to humans and the incomplete development of liver and kidney function in infants/fetuses, pregnant/lactating women should still use this product with caution. Precautions for the elderly: Due to the decline of physiological renal function, the dosage and medication interval of this product need to be adjusted.

Drug Interactions

1. When flucloxacillin and amoxicillin are used simultaneously with probenecid, the renal tubular secretion of flucloxacillin and amoxicillin can be reduced, resulting in increased blood drug concentrations and prolonged drug half-life. 2. The use of this product with live typhoid vaccine can reduce the immune effect of live typhoid vaccine. The possible mechanism is that this product has antibacterial activity against Salmonella typhi. 3. Since antibacterial drugs (chloramphenicol, erythromycin, sulfonamides, or tetracyclics) can interfere with the bactericidal effect of bactericidal drugs such as penicillins, it is best to avoid using this product in combination with antibacterial drugs when treating meningitis or other diseases that require rapid bactericidal effects. 4. This product may reduce the contraceptive effect of oral contraceptives containing estrogen. 5. The combined use of methotrexate and penicillins can reduce the clearance rate of methotrexate and cause poisoning; it is believed to be caused by competition for renal tubular secretion; the condition needs to be closely monitored; the dosage and duration of folinic acid need to be increased. 6. Allopurine uric acid synthesis inhibitors may increase the risk of adverse skin reactions of this drug.
