Function and Efficacy
The chemical structure of this product is similar to that of haloperidol and pethidine, but the therapeutic dose has no effect on the central nervous system. The effect on intestinal smooth muscle is similar to that of opioids. It can inhibit the contraction of intestinal smooth muscle and reduce intestinal peristalsis. It can also reduce the release of acetylcholine from intestinal nerve endings, and directly inhibit peristaltic reflexes through local interactions between cholinergic and non-cholinergic neurons. This product can prolong the residence time of food in the small intestine, promote the absorption of water, electrolytes and glucose, and inhibit intestinal over-secretion caused by prostaglandins, cholera toxin and other enterotoxins. In addition, this product can also increase the tension of the anal sphincter and inhibit fecal incontinence or urgency. LD50105mg/kg (oral for mice), 185mg (oral for rats).

Indication
For adults and children over 5 years old, this drug is used for: ① Symptomatic treatment of non-infectious acute and chronic diarrhea caused by various reasons. It can be used for patients with ileostomy to reduce the volume and frequency of defecation and increase the consistency of feces. It can also be used for patients after anorectal surgery to inhibit fecal incontinence.
Usage and Dosage
1 Acute diarrhea: Adults take 4 mg as the first dose, and then take 2 mg for each diarrhea event until the diarrhea stops or the dosage reaches 16 mg/day, and take it for 5 consecutive days. If it is ineffective, stop taking it; children over 5 years old take 2 mg as the first dose, and then take 2 mg for each diarrhea event until the diarrhea stops. The maximum dosage is 6 mg/day. Taking the medicine on an empty stomach or half an hour before meals can improve the efficacy. 2 Chronic diarrhea: The starting dose for adults is 2-4 mg, 2-12 mg per day, and 4-8 mg is given daily for maintenance treatment after the effect is obvious; children over 5 years old take 2 mg, and the dosage is adjusted according to the stool condition. Adults should not exceed 16 mg per day, and children should not exceed 3 mg/20 kg per day.
Adverse Reactions
Adverse reactions are mild and may include allergic reactions such as rashes, gastrointestinal symptoms such as dry mouth, abdominal distension, loss of appetite, gastrointestinal cramps, nausea, vomiting, constipation, as well as dizziness, headache, fatigue, etc.
Precautions
1. This product is contraindicated for children under 2 years old; 2. It is contraindicated for acute bacillary dysentery accompanied by high fever and bloody stools; 3. It is contraindicated for patients with pseudomembranous colitis caused by broad-spectrum antibiotics. 4. It should be used with caution in patients with severe liver damage.
Special Population Medication
Precautions for children: Children under 2 years old are prohibited. Precautions for pregnancy and lactation: Although this product has no teratogenic effect and embryotoxicity, pregnant women, especially those in the first three months of pregnancy, should still weigh the pros and cons before using it. This product can be secreted in breast milk in small amounts, so it is not suitable for lactating women. Precautions for the elderly: The medication for elderly patients is the same as that for adults.
