Function and Efficacy
This product belongs to the aminoglycoside antibiotics. The antibacterial spectrum is similar to that of gentamicin. It has antibacterial effects on Gram-negative bacteria such as Escherichia coli, aerogenes, Klebsiella, Proteus mirabilis, some indole-positive Proteus, Pseudomonas aeruginosa, some Neisseria, some non-pigmented Serratia and Shigella; the antibacterial effect of this product on Pseudomonas aeruginosa is 3 to 5 times stronger than that of gentamicin, and Pseudomonas aeruginosa that is moderately sensitive to gentamicin is highly sensitive to this product. Among Gram-positive bacteria, Staphylococcus aureus (including b-lactamase-producing strains) is sensitive to this product; Streptococci (including Streptococcus pyogenes, Pneumococcus, Streptococcus faecalis, etc.) are all resistant to this product. Anaerobic bacteria (Bacteroides), Mycobacterium tuberculosis, Rickettsia, viruses and fungi are also resistant to this product. The mechanism of action of this product is to bind to the 30S subunit of the bacterial ribosome and inhibit the synthesis of bacterial proteins.
Ingredients
The main ingredient of this product is tobramycin. Its chemical name is 0-3-amino-3-deoxy-α-D-glucopyranosyl-(1→4)-0-[2,6-dioxy-2,3,6-trideoxy-α-D-ribosyl-hexopyranosyl-(1→6)]-2-deoxy-L-streptamine. Its chemical structure is: Molecular formula: C18H37N5O9 Molecular weight: 467.52 Auxiliary materials are sodium chloride and sulfuric acid.

Appearance
This product is a colorless clear liquid.
Indication
This product is a sterile aqueous solution of tobramycin sulfate and sodium chloride.
Usage and Dosage
Intravenous drip. During the course of treatment, it is advisable to regularly measure the patient's blood peak and trough concentrations and adjust the dose accordingly. When treating severe infections, it is advisable to give the first shock dose to ensure that the drug quickly reaches an effective concentration in tissues and body fluids. The dose should be calculated based on standard body weight (removing excess fat). This product can be injected intragastrically or intravenously, with a course of 7 to 10 days. The dosage for patients with normal renal function is 2 to 3 mg/kg per day based on body weight, divided into 2 to 4 doses. For patients with severe infections, the dosage is 4 to 5 mg/kg per day based on body weight. After clinical symptoms improve, it should be reduced to 3 mg/kg per day. The dosage for infants and children is 3 to 5 mg/kg per day. For patients with renal dysfunction or the elderly, the first dose should be reduced or the dosing interval should be extended. When administering by intravenous drip, 80 mg of this product can be diluted to 50 or 100 ml with 5% glucose or normal saline, and then dripped within 20 to 60 minutes, with an interval of 6 to 8 hours.
Adverse Reactions
The adverse reactions are mainly toxic to the eighth cranial nerve and kidneys, and may include hearing loss, dizziness, vertigo, tinnitus, as well as symptoms of kidney damage such as proteinuria, tubular urine, increased blood urea nitrogen and blood creatinine.
Precautions
Patients who are allergic to this product or other aminoglycosides are contraindicated.
Special Population Medication
Precautions for children: Due to the potential toxicity of aminoglycoside antibiotics, patients should be closely observed during medication, especially those who may have renal disease or who use them for a long time. It should be used with caution in premature infants and newborns. Precautions for pregnancy and lactation: Pregnant and lactating women are prohibited. Precautions for the elderly: Elderly patients are prone to ear and kidney toxicity when using this product. If this product must be used, blood drug concentration, hearing and renal function should be closely monitored.
Drug Interactions
1. This product can increase ototoxicity, nephrotoxicity and neuromuscular blocking effects when used in combination with other aminoglycosides. Hearing loss may occur, and may still progress to deafness after discontinuation of the drug; hearing damage may recover or be permanent. Neuromuscular blocking effects can lead to skeletal muscle weakness, respiratory depression or respiratory paralysis (apnea), and anticholinesterase drugs or calcium salts can help restore the blocking effect. 2. This product can aggravate neuromuscular blocking effects when used in combination with neuromuscular blocking drugs, leading to muscle weakness, respiratory depression or respiratory paralysis (apnea). When used in combination with plasma substitutes such as dextran, sodium alginate, diuretics such as ethacrynic acid, furosemide, capreomycin, cisplatin, vancomycin, etc., or when used locally or systemically in sequence, it can increase ototoxicity and nephrotoxicity, and hearing damage may occur, and may still develop to deafness after discontinuation of the drug, and hearing damage may recover or be permanent. 3. This product can increase nephrotoxicity when used in combination with cephalothin. 4. This product can increase nephrotoxicity and neuromuscular blocking effects when used in combination with polymyxins. Neuromuscular blocking effects can lead to skeletal muscle weakness, respiratory depression or respiratory paralysis (apnea). 5. This product should not be used in combination with other nephrotoxic or ototoxic drugs or used sequentially to avoid aggravating nephrotoxicity or ototoxicity.
