Clindamycin Palmitate Hydrochloride Granules
Function and Efficacy
This product is a derivative of clindamycin. It has no antibacterial activity in vitro. It is hydrolyzed by esterase in vivo to form clindamycin and exert its antibacterial activity. The mechanism of action of clindamycin is to inhibit the synthesis of bacterial proteins. It mainly acts on Gram-positive cocci and anaerobic infections. This product has good antibacterial effects on Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus pneumoniae, Streptococcus pyogenes, etc., and is moderately sensitive to Haemophilus influenzae and Neisseria gonorrhoeae. Some erythromycin-resistant Staphylococcus aureus and Staphylococcus epidermidis are still sensitive to this product. This product has good antibacterial effects on both Gram-negative and Gram-positive anaerobic bacteria. The MIC50 and MIC90 for Bacteroides fragilis are 0.062 and 0.5 mg/ml respectively, and the MIC50 and MIC90 for Peptostreptococcus are 0.125 and 4 mg/ml respectively.
Ingredients
The main ingredient of this drug is clindamycin palmitate hydrochloride, whose chemical name is 6-1(1-methyl-trans-4-propyl-L-2-pyrrolidinecarboxamido)-1-thio-7(S)-chloro-6,7,8-trideoxy-L-threo-a-D-galactoctinopyranoside-2-palmitate hydrochloride.

Appearance
This product is pink granules with aromatic smell and sweet taste.
Indication
1. For the following infectious diseases caused by Gram-positive bacteria: (1) tonsillitis, suppurative otitis media, sinusitis, etc.; (2) acute bronchitis, acute exacerbation of chronic bronchitis, pneumonia, lung abscess and bronchiectasis combined with infection, etc.; (3) skin and soft tissue infections: furuncle, carbuncle, abscess, cellulitis, trauma, burns and postoperative infection, etc.; (4) urinary tract infections: acute urethritis, acute pyelonephritis, prostatitis, etc.; (5) others: osteomyelitis, sepsis, peritonitis and oral infection, etc.; 2. Usage and Dosage
Oral: Add 45ml of water to each bottle and shake to dissolve before taking (60ml after dissolution, 5ml is equivalent to 75mg of clindamycin). According to clindamycin, 1. Children: daily dose is 8-25mg/kg according to body weight (8-16mg/kg for general infection, 17-25mg/kg for severe infection), divided into 3-4 doses (children weighing less than 10kg should take no less than 37.5mg each time, 3 times a day). 2. Adults: 150-300mg at a time (450mg can be used for severe infection), 4 times a day. Or follow the doctor's advice.
Adverse Reactions
1. Gastrointestinal reactions: Occasionally nausea, vomiting, abdominal pain and diarrhea. 2. Allergic reactions: A few patients may develop drug-induced rashes. 3. There is basically no toxic reaction to the hematopoietic system, but it may occasionally cause neutropenia, eosinophilia, thrombocytopenia, etc., which are generally mild and transient. 4. A few patients may experience transient alkaline phosphatase, mild elevation of serum transaminases and jaundice. 5. A very small number of patients may develop pseudomembranous conjunctivitis.
Precautions
1. It is contraindicated for those who are allergic to lincomycins and lincomycin. 2. It should be used with caution in pregnant and lactating women. 3. It should be used with caution in preterm infants.

Drug Interactions
1. This product can enhance neuromuscular blockade and respiratory depression when used in combination with neuromuscular blockers, central anesthetics, and opioid analgesics with respiratory depression; while it can weaken the antimuscular effect when used in combination with antimuscular relaxants. 2. Chloramphenicol and erythromycin can replace or prevent the binding of clindamycin to the bacterial ribosome 50S subunit, so they should not be used in combination with this product. 3. This product may enhance the antibacterial effect when used in combination with gentamicin and colistin.
