Basic Information
- Chemical Name: Dopamine Hydrochloride, with the chemical formula .
- Appearance: It is a white or off - white crystalline powder with a luster.
- Odor: Almost odorless, with a slightly bitter taste.
- Solubility: It is freely soluble in water, slightly soluble in absolute ethanol, and very slightly soluble in chloroform or ether.
Pharmacological Actions
- Mechanism of Action: It is an endogenous catecholamine that mainly stimulates α, β, and peripheral dopamine receptors. At a low dose (0.5 - 2μg/kg per minute based on body weight), it mainly acts on dopamine receptors, dilating the renal and mesenteric blood vessels, increasing renal blood flow and glomerular filtration rate, and increasing urine output and sodium excretion. At a medium dose (2 - 10μg/kg per minute based on body weight), it can directly stimulate β₁ receptors, producing a positive inotropic effect on the myocardium, strengthening myocardial contractility, and increasing cardiac output. At a high dose (more than 10μg/kg per minute based on body weight), it stimulates α - receptors, leading to an increase in peripheral vascular resistance, renal vasoconstriction, and a decrease in renal blood flow and urine output instead.
- Metabolic Process in the Body: It is rapidly metabolized and inactivated by monoamine oxidase (MAO) and catechol - O - methyltransferase (COMT) in the body. The main metabolites are 3,4 - dihydroxy - phenylacetic acid and 3 - methoxy - 4 - hydroxy - phenylacetic acid.
Clinical Applications
- Treatment of Shock: It is used for various types of shock, such as septic shock, cardiogenic shock, and hemorrhagic shock. It is especially suitable for shock patients with weakened myocardial contractility and decreased urine output when the blood volume has been replenished. It can increase cardiac output and improve renal blood flow and renal function.
- Treatment of Acute Renal Failure: When combined with diuretics, it can increase urine output, improve renal function, and prevent acute tubular necrosis.
Administration and Dosage
- Intravenous Infusion: Generally, Dopamine Hydrochloride is added to glucose injection or sodium chloride injection for dilution and then given by intravenous infusion. The specific dosage is determined according to factors such as the patient's condition, age, and body weight. For example, when treating shock, the infusion is usually started at a rate of 2 - 5μg/kg per minute, and the dosage is adjusted according to blood pressure, heart rate, urine output, and other conditions.
- Precautions: It must be diluted before infusion, and a relatively thick vein should be selected for injection to prevent tissue necrosis caused by drug extravasation. During the infusion process, the patient's vital signs such as blood pressure, heart rate, and urine output should be closely monitored.
Adverse Reactions and Contraindications
- Adverse Reactions
  - Cardiovascular System: Common symptoms include chest pain, dyspnea, palpitations, and arrhythmias. High doses can lead to elevated blood pressure and induce ventricular arrhythmias.
  - Peripheral Tissues: If the drug extravasates, it can cause local tissue ischemia and necrosis.
