    Olaparib, also known as AZD-2281 or KU-59436 , is a small molecule inhibitor of the nuclear enzyme poly(ADP-ribose) polymerase (PARP) with potential chemosensitizing, radiosensitizing, and antineoplastic activities. Olaparib selectively binds to and inhibits PARP, inhibiting PARP-mediated repair of single strand DNA breaks; PARP inhibition may enhance the cytotoxicity of DNA-damaging agents and may reverse tumor cell chemoresistance and radioresistance.

    1. What is Olaparib?

    Olaparib is an oral anticancer medication used to treat certain types of cancers that are associated with defective DNA repair mechanisms, particularly in patients with BRCA1 or BRCA2 mutations. It belongs to a class of targeted therapies known as PARP inhibitors. Olaparib helps prevent cancer cells from repairing their DNA, leading to cell death — especially in cancer cells already impaired in DNA repair.

    4. Mechanism of Action:

    Olaparib inhibits PARP enzymes (mainly PARP-1 and PARP-2), which are involved in repairing single-strand breaks in DNA. In tumors with BRCA1 or BRCA2 mutations, the homologous recombination repair (HRR) pathway is defective. Olaparib blocks the backup DNA repair pathway (mediated by PARP), leading to accumulation of DNA damage, genomic instability, and ultimately cancer cell death. This mechanism is called synthetic lethality.

    5. Approved Indications (Uses):

    As of the latest FDA and EMA approvals, Olaparib is indicated for treatment of the following cancers:

    A. Ovarian Cancer:

    Maintenance treatment of adult patients with recurrent epithelial ovarian, fallopian tube, or primary peritoneal cancer who are in complete or partial response to platinum-based chemotherapy

    Maintenance treatment in newly diagnosed, advanced BRCA-mutant ovarian cancer

    Treatment of BRCA-mutated advanced ovarian cancer after 3 or more lines of chemotherapy

    B. Breast Cancer:

    Germline BRCA-mutated, HER2-negative metastatic breast cancer; previously treated with chemotherapy (and endocrine therapy if hormone receptor-positive)

    C. Pancreatic Cancer:

    Germline BRCA-mutated metastatic pancreatic adenocarcinoma; maintenance treatment after at least 16 weeks of initial platinum-based chemotherapy without disease progression

    D. Prostate Cancer:

    Metastatic castration-resistant prostate cancer (mCRPC) with homologous recombination repair (HRR) gene mutations (e.g., BRCA1, BRCA2, ATM), after prior treatment with enzalutamide or abiraterone

    6. Dosage and Administration:

    General adult dosing (may vary by indication):

    300 mg orally twice daily (total daily dose: 600 mg)

    Continue until disease progression or unacceptable toxicity

    Administration tips:

    Take with or without food

    Swallow tablets whole; do not chew, crush, or split

    Avoid taking with grapefruit or Seville oranges (may increase drug levels)

    Different formulations:

    Tablets and capsules are NOT interchangeable (due to differences in formulation and bioavailability)

    Risk of MDS/AML

    Embryo-fetal toxicity: Avoid in pregnancy; use effective contraception during treatment and for 6 months after (females)

    Male patients with partners of childbearing potential should use contraception during and for 3 months after the last dose
