Erythromycin is a macrolide antibiotic used to treat a variety of bacterial infections by inhibiting bacterial protein synthesis.

Mechanism of Action

Erythromycin binds to the 23S rRNA component of the 50S subunit of bacterial ribosomes, blocking the transpeptidation and translocation steps of protein synthesis. This prevents bacterial growth by inhibiting elongation of the peptide chain. It does not affect nucleic acid synthesis. Its strong affinity for bacterial ribosomes underlies its broad-spectrum antibacterial activity.

Applications

Erythromycin is indicated for treatment of infections caused by susceptible bacteria, including:
Respiratory tract infections: Mild to moderate upper and lower respiratory infections caused by Streptococcus pyogenes, Streptococcus pneumoniae, Haemophilus influenzae (often combined with sulfonamides), Mycoplasma pneumoniae, Bordetella pertussis (whooping cough), Listeria monocytogenes, Corynebacterium diphtheriae (diphtheria), and possibly Legionella species.
Skin infections: Mild to moderate infections caused by Streptococcus pyogenes, Staphylococcus aureus, and Corynebacterium minutissimum (erythrasma).
Gastrointestinal infections: Intestinal amebiasis caused by Entamoeba histolytica.
Genital infections/STIs: Alternative treatment for pelvic inflammatory disease caused by Neisseria gonorrhoeae in penicillin-allergic patients, primary syphilis (Treponema pallidum), chlamydial infections including neonatal conjunctivitis and pneumonia, nongonococcal urethritis caused by Chlamydia trachomatis and Ureaplasma urealyticum.

Routes of Administration

Oral: Available as base, estolate, stearate, or ethylsuccinate forms in tablets, capsules, suspensions, and delayed-release formulations. Absorption is variable (18-45%) and sensitive to acidic degradation; some formulations are enteric-coated to improve stability.
Intravenous: For severe infections.

Pharmacokinetics

Absorption: Oral erythromycin is variably absorbed, with peak plasma concentrations reached approximately 1 to 1.5 hours post-dose. Food may reduce absorption for some formulations but not all.
Distribution: Widely distributed in body fluids and tissues, including accumulation in leukocytes and inflammatory exudates. Crosses the placenta; cerebrospinal fluid penetration is low but increases with inflamed meninges.
Protein Binding: Approximately 80-93% bound to serum proteins.
Metabolism: Extensively metabolized in the liver by CYP3A4 to active and inactive metabolites, including anhydroerythromycin, which contributes to drug interactions by inhibiting hepatic enzymes.
Elimination: Mainly excreted in bile; less than 5% excreted unchanged in urine.
Half-life: Approximately 1.5 to 2 hours, necessitating multiple daily doses.

Safety and Side Effects

Common Side Effects: Gastrointestinal disturbances such as abdominal cramping, nausea, vomiting, diarrhea, and flatulence. These are dose-related and may be due to local irritation or motilin receptor agonism.
Serious Side Effects: Pseudomembranous colitis, hepatotoxicity (cholestatic jaundice, elevated liver enzymes), allergic reactions (rash, urticaria, anaphylaxis), hearing loss (rare), cardiac arrhythmias including QT prolongation and torsades de pointes.
Special Populations: Infantile hypertrophic pyloric stenosis reported in infants; caution advised in neonates and young infants.
Drug Interactions: Strong inhibitor of CYP3A4, leading to increased levels of many drugs (e.g., statins, warfarin, theophylline). The inactive metabolite anhydroerythromycin contributes to these interactions.
Monitoring: Liver function tests and ECG monitoring may be warranted in high-risk patients.
