Introduction:

Primary Function: Cardiac Myosin Inhibitor

• Directly Target Cardiac Myosin:​ It binds to the protein myosin in heart muscle cells (cardiomyocytes), specifically to the S1 domain of beta-cardiac myosin.
• Reduce Hypercontractility:​ By binding to myosin, it decreases the number of active myosin heads available to interact with actin. This leads to a dose-dependent reduction in the excessive force generation (hypercontractility)​ and impaired relaxation that are hallmarks of certain heart diseases.
• Improve Energetics:​ It is believed to lower the heart's excessive energy demand and improve cardiac efficiency by stabilizing myosin in a low-energy state.

Primary Application: Treatment of Obstructive Hypertrophic Cardiomyopathy (oHCM)

• Mechanism in oHCM:​ In oHCM, a thickened heart muscle (septum) obstructs blood flow out of the left ventricle. The hypercontractility worsens this obstruction. Aficamten reduces this hypercontractility, thereby:
