The Current State Of Research On The Best Erectile Dysfunction Pills
Erectile dysfunction (ED) is the inability of a man to consistently obtain and maintain a sufficient penile erection for satisfactory sex. Other types of sexual dysfunction, such as premature ejaculation and low libido, sometimes occur, however, the most common and devastating problem in men is ED.
One study showed that 24% of men had testosterone levels below 300ng/dL. Diseases such as hypertension, diabetes, and cardiovascular disease, as well as mental illnesses such as depression and anxiety, contribute to sexual dysfunction in middle-aged and older men.
Phosphodiesterase -5 inhibitor(PDE5)are generally considered the best erectile dysfunction pills. In ED patients with low testosterone levels, androgen therapy can improve erectile function in patients who do not respond to PDE5 inhibitors, and there may be a synergistic effect when combined with PDE5 inhibitors. But ED patients with diabetes and ED or radical prostatectomy did not respond well to these drugs. Therefore, there is a need to find new therapeutic strategies that overcome the deficiencies of currently available oral drugs. This article describes the best erectile dysfunction pills treatments in clinical or preclinical research, including targeted drug therapy, protein therapy and gene therapy.
Targeted drug therapy
While oral PDE5 inhibitors are effective in most patients, they are not satisfactory for all men with ED. Therefore, with the approval of apomorphine for the treatment of ED, many researchers have attempted to develop new drugs targeting different molecular pathways in the central and peripheral regions. Centrally targeted ones include melanocortin receptor agonists, dopaminergic agonists, and clavulanic acid. Peripheral targets include soluble guanylate cyclase activators and Rho kinase inhibitors. Melanocortin receptor agonists, such as melanin II and bumenotide, have been evaluated for efficacy and safety in patients with ED. However, a significant proportion of patients also experienced adverse events such as severe nausea and yawning; Intranasal administration of bumenotide significantly improves erectile function in diabetic ED patients or in patients who do not respond to PDE5 inhibitor therapy. Common side effects include nausea and high blood pressure; Soluble guanylate cyclase activators and Rho kinase inhibitors have been shown to improve erectile function in some clinical trials. However, further development was halted as it did not show superiority over currently available oral PDE5 inhibitors. Therefore, it cannot be used as the best erectile dysfunction pills.
Protein therapy and gene therapy
Proteins in penile tissue in ED patients may not function properly, and replacing these proteins may improve erectile function. Protein therapy and gene therapy aim to deliver the correct proteins or genes that make proteins and related products.
Angiogenesis
The penis is an organ rich in blood vessels, and the vascular endothelium plays an important role in regulating vascular tone and blood flow. ED is often directly associated with endothelial dysfunction. Endothelial cells can be damaged through a variety of mechanisms, most of which lead to oxidative stress in the tissue. Many causes of oxidative stress are related to lifestyles that contribute to high blood pressure, diabetes, and dyslipidemia. Endothelial cell dysfunction results in decreased endothelium-dependent vasodilation and increased leukocyte adhesion to the endothelium. Endothelial cell damage results in various sequelae, including ED, other types of vasoconstriction, atherosclerosis, and thrombosis. Therefore, re-establishing the microvasculature of penile tissue and restoring endothelial function may treat the underlying conditions of ED and overcome the limitations of oral PDE5 inhibitors.
Neuroregeneration
The cavernous nerve is the main autonomic innervation of the penis. Cavernous nerve injury(CNI)is the main cause of ED after radical prostatectomy. Even if bilateral nerve-sparing methods and minimally invasive methods are used for treatment, a certain degree of CNI is inevitable. Restoration of cavernosal nerve function is the ultimate goal of neuroprotective or regenerative strategies. Neurotrophic factors are a family of proteins that induce neuronal survival, development and function. Neurotrophic factors including brain-derived nerve growth factor, neurotrophic factor-3, glial cell line-derived neurotrophic factor and growth differentiation factor-5, partially restored by protection of cavernous nerves or nerve regeneration in animal models of ED erectile function.
Summary
PDE5 inhibitors and androgen therapy for ED are currently the best erectile dysfunction pills treatment methods, but some patients do not respond well to these drugs. Many preclinical studies based on protein therapy and gene therapy have shown that it is possible to restore erectile function by regenerating cavernous vascular endothelial cells and cavernous nerves and inhibiting cavernous fibrosis. But because of the complexity of protein engineering and genetic engineering, it has not yet entered the clinical trial stage. ED has multiple pathogenic mechanisms, and the combined application of multiple drug treatment strategies may achieve better results. The current preclinical data also provide a solid foundation for the best erectile dysfunction pills in the future.
