Cefoperazone is a third-generation cephalosporin, which has good antibacterial effect on Escherichia coli, Klebsiella, Proteus, Salmonella typhi, Shigella, Citrobacter and other Enterobacteriaceae and Pseudomonas aeruginosa, but has poor effect on Enterobacter aerogenes, Enterobacter cloacae, Salmonella typhimurium and Acinetobacter. Haemophilus influenzae, Neisseria gonorrhoeae and Neisseria meningitidis are highly sensitive to this product.

The MIC for Staphylococcus aureus is 2 to 4 mg/L. Methicillin-resistant Staphylococci and enterococci are resistant to this product. Most Bacteroides fragilis are resistant to this product.
