Phenylephrine Hydrochloride Injection
Function and Efficacy
This product is an α-adrenaline receptor agonist. This product is a sympathomimetic amine that acts directly on the receptor, but sometimes it also works indirectly by promoting the release of norepinephrine from the storage site. It acts on α receptors (especially the skin, mucous membranes, internal organs, etc.), causing vasoconstriction, increased peripheral resistance, and increased systolic and diastolic blood pressure. As blood pressure rises, it can stimulate the vagus nerve reflex and slow the heart rate, thereby treating supraventricular tachycardia. This product has a longer vasoconstrictive effect than adrenaline or ephedrine, and rarely causes central nervous system excitation at therapeutic doses; this product reduces blood flow to the kidneys, internal organs, skin and limbs, but increases coronary blood flow. As a vasoconstrictor, adding local anesthetic solution can slow down the latter's absorption, thereby limiting the scope of local anesthesia and prolonging its duration.

Appearance
This product is a colorless clear liquid.
Indication
Used to treat shock and maintain blood pressure during anesthesia. Also used to treat supraventricular tachycardia.
Usage and Dosage
Common dosage for adults: 1. For vasoconstriction, 1 mg of this product can be added to every 20 ml of local anesthetic solution to reach a concentration of 1:20000; for subarachnoid blockade, every 2-3 ml can reach a concentration of 1:1000. 2. For hypertension, mild or moderate hypotension, 2-5 mg can be injected intramuscularly, and the interval between re-administration should not be shorter than 10-15 minutes. 0.2 mg can be injected intravenously once, and the dosage can be administered every 10-15 minutes as needed. 3. For paroxysmal supraventricular tachycardia, the initial dose is 0.5 mg intravenously, injected within 20-30 seconds, and the dosage is increased gradually thereafter, and the dosage added each time should not exceed 0.1-0.2 mg, and the dosage is limited to 1 mg at a time. 4. For severe hypotension and shock (including drug-related hypotension), intravenous administration can be used. 10 mg of this product can be added to every 500 ml of 5% glucose injection or 0.9% sodium chloride injection (1:50,000 concentration). The initial drip rate is 100-180 drops per minute. After the blood pressure stabilizes, the drip rate is gradually reduced to 40-60 drops per minute. If necessary, the concentration can be doubled and the drip rate is adjusted according to the blood pressure. (5) In order to prevent hypotension during subarachnoid block, 2-3 mg of this product can be injected intramuscularly 3-4 minutes before the block.
Adverse Reactions
1. Chest discomfort or pain, dizziness, irritability, tremor, dyspnea, weakness, etc. are generally rare, but should be paid attention to when they persist. 2. Persistent headaches and abnormally slow heart rate, vomiting, head swelling or numbness and tingling in the hands and feet indicate that blood pressure is too high and overdose should be taken seriously immediately and the dosage should be adjusted; reflex bradycardia can be corrected with atropine, and other overdose symptoms can be treated with alpha receptor blockers such as phentolamine. 3. When intravenous medication is used to treat paroxysmal tachycardia, the heart rate often increases or becomes irregular, indicating an overdose.
Precautions
It is contraindicated for patients with hypertension, coronary artery sclerosis, hyperthyroidism, diabetes, myocardial infarction, or those who have used monoamine oxidase inhibitors within the past two weeks.
Special Population Medication
Precautions for children: There is still a lack of research on the use of this product in children. Precautions for pregnancy and lactation: Animal experiments have found that it is fetotoxic. Use in late pregnancy or during delivery can increase uterine contractions, reduce blood flow, and cause fetal hypoxia and bradycardia. Therefore, pregnant women should avoid using it unless necessary. Precautions for the elderly: Use with caution in the elderly to avoid causing severe bradycardia or (and) reduced cardiac output, and the dosage should be appropriately reduced.
Drug Interactions
1. When the drug is administered after taking a receptor blocker such as phentolamine, phenoxybenzamine, tolazoline, phenothiazines, etc., the pressor effect of this product may be weakened. 2. When used with general anesthetics (especially cyclopropane or halogenated hydrocarbons), it is easy to cause ventricular arrhythmias; it is also not advisable to add this product to the local anesthetic solution and use it at the ends of the fingers and toes to avoid extreme contraction of the peripheral blood vessels and cause tissue necrosis and ulcers. 3. When used with antihypertensive drugs, the antihypertensive effect may be weakened. 4. When used with guanethidine, the effect of guanethidine may be reduced and the pressor effect of this product may be enhanced. 5. When used with oxytocin, it may cause severe hypertension. 6. When used with monoamine oxidase (MAO) inhibitors, the pressor effect of this product may be enhanced. This product is prohibited within 14 days after using MAO inhibitors. 7. When used with sympathomimetic drugs, the potential adverse reactions of this type of drug may be easily manifested. 8. When used with thyroid hormones, the effects of both drugs are enhanced. 9. The pressor effect of this product is enhanced when used with tricyclic antidepressants. 10. When used with nitrates, the pressor effect of this product and the anti-anginal effect of nitrates are weakened.
