Function and Efficacy

This product is a histamine H2 receptor antagonist, which has the effect of inhibiting gastric acid secretion. It has a strong acid-suppressing effect and can effectively inhibit the basic gastric acid secretion and gastric acid secretion caused by various reasons such as food, histamine, pentagastrin, caffeine and insulin, so as to reduce the amount of secretion and acidity, and can prevent or reduce the corrosive damage of gastric mucosa caused by bile salts, alcohol, aspirin and other non-steroidal anti-inflammatory drugs, and has obvious curative effect on stress ulcer and upper gastrointestinal bleeding. In addition, this product has anti-androgen effect and has certain value in the treatment of hirsutism; it can also weaken the activity of immunosuppressive cells, enhance immune response, thereby inhibiting tumor metastasis and prolonging survival. According to foreign data reports, this product can also effectively treat acute intermittent porphyria. Histamine activates adenylate cyclase by exciting H2 receptors, increasing the production of cAMP in gastric wall cells, and cAMP activates carbonic anhydrase through protein kinase, catalyzing CO2 and H2O to produce H2CO3, and further dissociates to release H, which increases gastric acid secretion. Cimetidine mainly acts on the H2 receptors on the parietal cells, competitively inhibiting histamine, thus inhibiting gastric acid secretion. This product has a significant inhibitory effect on gastric acid secretion, can significantly inhibit basal and nocturnal gastric acid secretion, can also inhibit gastric acid secretion caused by stimulation such as histamine, gastrin peptide, insulin and food, and reduce its acidity. It has a preventive and protective effect on corrosive gastritis caused by chemical stimulation, and also has a significant therapeutic effect on liquid gastric ulcers and upper gastrointestinal bleeding.

Drug Interactions

When used in combination with warfarin, theophylline, phenytoin and other drugs, the dosage should be reduced. Antacids such as aluminum hydroxide or metoclopramide can reduce the absorption of this product. It can reduce the effects of sucralfate, tetracycline, etc. When used in combination with opioids, patients with chronic renal failure may experience respiratory depression, mental confusion and disorientation, etc. The dosage of opioid preparations should be reduced. 1 This product is a liver drug enzyme inhibitor, which reduces the activity of drug enzymes by binding to cytochrome P450 through its imidazole ring, and can also reduce liver blood flow. Therefore, when this product is used in combination with propranolol, the latter's blood concentration can be increased and the heart rate at rest can be slowed down; when it is used in combination with phenytoin sodium or other hydantoins, the latter's blood concentration can be increased, which may lead to phenytoin sodium poisoning. If it must be used in combination, the blood concentration of phenytoin sodium should be measured after 5 days in order to adjust the dosage; 2. When this product is used in combination with cyclosporine, the latter's blood concentration can be increased; 3. When this product is used in combination with moclobemide, the latter's blood concentration can be increased; 4. When this product is used in combination with aspirin, the solubility of aspirin can be increased. The solubility is increased, the absorption is increased, and the effect is enhanced; 5 When this product is used in combination with methadone, the latter's blood concentration can be increased, which may cause an overdose; 6 When this product is used in combination with tacrine, the latter's blood concentration can be increased, which may cause an overdose; 7 When this product is used in combination with benzodiazepines (such as diazepam, nitrazepam, flunitrazepam, chlordiazepoxide, midazolam, triazolam, etc.), it can inhibit the latter's liver metabolism, increase its blood concentration, aggravate its sedation and other central nervous system depression symptoms, and may develop into respiratory and circulatory failure. However, lorazepam, oxazepam and temazepam seem to be unaffected; 8 When this product is used in combination with coumarin anticoagulants, the latter's excretion rate from the body can be reduced, and the prothrombin time can be further prolonged, thereby leading to a bleeding tendency. When the two are used together, it is necessary to pay close attention to changes in the condition and adjust the dosage of anticoagulants; 9 This product can increase the absolute bioavailability of verapamil (Isopamine). Since verapamil can cause serious adverse reactions, although rare, it should still be paid attention to; 10 When this product is used in combination with theophylline, the latter's demethylation metabolic clearance rate can be reduced by 20-30%, and the blood concentration of the drug increases; 11 Patients who take digoxin and quinidine at the same time should not use this product together, because this product can inhibit the metabolism of quinidine, and the latter can displace digoxin from its binding site, resulting in an increase in the blood concentration of quinidine and digoxin; 12 When this product is used in combination with carbamazepine, the latter's blood concentration can be increased, which may lead to the risk of overdose; 13 When this product is used in combination with lidocaine (extra-gastrointestinal medication), the latter's blood concentration can be increased, thereby increasing the risk of adverse reactions to the nervous system and heart. When the two are used together, the lidocaine dosage needs to be adjusted and clinical monitoring needs to be strengthened; 14 When this product is used in combination with caffeine, it can delay the metabolism of caffeine, strengthen its effect, and easily cause toxic reactions. Patients with gastric ulcers should avoid caffeine, and caffeine and caffeinated beverages should be prohibited when taking this product; 15 When this product is used in combination with antacids (such as aluminum hydroxide and magnesium oxide), it can relieve the pain of duodenal ulcers, but the absorption of cimetidine may be reduced, so it is generally not recommended to use the two together. If they must be used together, the two should be taken at least 1 hour apart; 16 When used in combination with metoclopramide, the blood concentration of this product can be reduced. If the two need to be used together, the dosage of this product should be appropriately increased; 17 Since sucralfate needs to be hydrolyzed by gastric acid to work, and this product inhibits gastric acid secretion, the efficacy of sucralfate may be reduced when the two are used together; 18 This product increases the pH value of gastric juice. When used in combination with tetracycline, it can reduce the dissolution rate of tetracycline, reduce absorption, and weaken its effect; but the hepatic enzyme inhibition effect of this product may increase the blood concentration of tetracycline; 19 When this product is used in combination with ketoconazole, it can interfere with the absorption of the latter and reduce its antifungal activity, but taking some acidic beverages at the same time can avoid the above changes. 20 This product may cause psychotic symptoms when used in combination with captopril; 21 Since this product has a neuromuscular blocking effect similar to that of aminoglycosides, it may cause respiratory depression or respiratory arrest when used in combination with aminoglycoside antibiotics; 22 This product should be avoided from being used simultaneously with central anticholinergics to prevent aggravation of central nervous system toxicity; 23 When this product is used in combination with carmustine, it may increase bone marrow toxicity; 24 When this product is used in combination with opioids, there have been reports of adverse reactions such as respiratory depression, mental confusion, and disorientation in patients with chronic renal failure;
