Function and Efficacy

This product is a competitive angiotensin converting enzyme inhibitor, which prevents angiotensin I from converting into angiotensin II, thereby reducing peripheral vascular resistance and reducing water and sodium retention by inhibiting aldosterone secretion. This product can also dilate peripheral blood vessels by interfering with the degradation of bradykinin. For patients with heart failure, this product can also reduce pulmonary capillary wedge pressure and pulmonary vascular resistance, increase cardiac output and exercise tolerance time.

Appearance

This product is a white or off-white sugar-coated tablet, which appears white or off-white after removing the sugar coating.

Indication

(1) Hypertension. (2) Heart failure.

Usage and Dosage

It depends on the condition or individual differences. This product should be taken under the guidance or supervision of a physician. The dosage must follow the principle of individualization and be adjusted according to the efficacy. 1 Common dosage for adults (1) For hypertension, take 12.5 mg orally once, 2 to 3 times a day. Increase to 50 mg 2 to 3 times a day within 1 to 2 weeks as needed. If the efficacy is still unsatisfactory, other antihypertensive drugs can be added. (2) For heart failure, start with 12.5 mg orally once, 2 to 3 times a day. If necessary, gradually increase to 50 mg 2 to 3 times a day. If further increase is needed, observe the efficacy for 2 weeks before considering it. For patients who have recently taken a large amount of diuretics and are in hyponatremia/hypovolemia, but have normal or low blood pressure, the initial dose should be 6.25 mg 3 times a day, and then gradually increase to the common dosage through testing. 2 Common dosage for children to reduce blood pressure and treat heart failure, both start with 0.3 mg/kg of body weight, 3 times a day. If necessary, increase by 0.3 mg/kg every 8 to 24 hours to obtain the lowest effective dose.

Adverse Reactions

1 The more common ones are: ① Rash, which may be accompanied by itching and fever, often occurs within 4 weeks of treatment, presents as maculopapular rash or urticaria, disappears after dose reduction, drug discontinuation or administration of antihistamines, 7% to 10% are accompanied by eosinophilia or positive antinuclear antibodies; ② Palpitations, tachycardia, chest pain; ③ Cough; ④ Difficulty in taste. 2 The less common ones are: ① Proteinuria, which often occurs within 8 months of the start of treatment, of which 1/4 develop nephrotic syndrome, but proteinuria gradually decreases within 6 months and the course of treatment is not affected; ② Dizziness, headache, and fainting. Caused by hypotension, especially in the absence of sodium or hypovolemia; ③ Angioedema, seen on the face, hands and feet; ④ Rapid and irregular heart rate; ⑤ Facial flushing or pallor. 3 The rare ones are: Leukopenia and granulocytopenia, with fever, chills, and leukopenia

Precautions

Patients who are allergic to this product or other angiotensin-converting enzyme inhibitors are contraindicated.

Special Population Medication

Precautions for children: There have been reports that this product can cause excessive and persistent decreases in blood pressure in infants, accompanied by oliguria and convulsions, so the use of this product is limited to those who have not responded to other antihypertensive treatments. Precautions for pregnancy and lactation: 1. This product can pass through the placenta. 2. This product can be excreted into breast milk, and its concentration is about 1% of the maternal blood drug concentration, so lactating women must weigh the pros and cons when using it. 3. Pregnant women who absorb ACEⅠ can affect fetal development and even cause fetal death, so pregnant women are prohibited from using it. Precautions for the elderly: The elderly are more sensitive to antihypertensive effects, so the dosage of this product should be reduced as appropriate.

Drug Interactions

1. When used with diuretics, the antihypertensive effect is enhanced, but severe hypotension should be avoided. Therefore, those who originally used diuretics should stop taking them or reduce the dosage. This product should be started with a small dose and the dosage should be adjusted gradually. 2. When used with other vasodilators, it may cause hypotension. If used together, it should be started with a small dose. 3. When used with potassium-retaining drugs such as spironolactone, triamterene, and amiloride, it may cause hyperkalemia. 4. When used with endogenous prostaglandin synthesis inhibitors such as indomethacin, the antihypertensive effect of this product will be weakened. 5. When used with other antihypertensive drugs, the antihypertensive effect is enhanced; it has an additive effect with drugs that cause renin release or affect sympathetic activity; it has a less than additive effect with b-blockers.
