Function and Efficacy
Kanamycin sulfate is an aminoglycoside antibiotic. It has good antibacterial effects on most Enterobacteriaceae such as Escherichia coli, Klebsiella, Enterobacter, Proteus, Shigella, Salmonella, Citrobacter, Providencia, Yersinia, etc.; influenza bacillus, Brucella, meningococci, gonococci, etc. are also mostly sensitive to this product. Kanamycin also has a certain effect on Staphylococcus (methicillin-sensitive strains) and Mycobacterium tuberculosis, but is ineffective against Pseudomonas aeruginosa. Other Gram-positive bacteria such as hemolytic streptococci, Streptococcus pneumoniae, Enterococcus and anaerobic bacteria are mostly resistant to this product. This product mainly binds to the 30S subunit of the bacterial ribosome and inhibits bacterial protein synthesis. In recent years, the number of resistant strains has increased significantly, because some bacteria produce aminoglycoside inactivating enzymes, which make them lose their antibacterial activity. Kanamycin has complete cross-resistance with streptomycin and neomycin, and may have partial cross-resistance with other aminoglycosides.

Appearance
This product is a colorless or slightly yellow clear liquid.
Indication
This product is suitable for the treatment of serious infections such as pneumonia, sepsis, abdominal infection, etc. caused by sensitive Enterobacteriaceae such as Escherichia coli, Klebsiella, Proteus, Enterobacter aerogenes, Shigella, etc. The latter two often need to be used in combination with other antibacterial drugs.
Usage and Dosage
1 The common dose for adults is intramuscular injection or intravenous drip, 0.5g at a time, once every 12 hours; or 7.5mg/kg per day according to body weight, once every 12 hours. The daily dose for adults should not exceed 1.5g, and the course of treatment should not exceed 14 days. The dose should be appropriately reduced for patients over 50 years old. 2 The common dose for children is intramuscular injection or intravenous drip, 15-25mg/kg per day according to body weight, administered in 2 doses. 3 Dosage for renal impairment: When creatinine clearance is 50-90mg/min, use 60-90% of the normal dose, once every 12 hours (normal dose is 7.5mg/kg each time, once every 12 hours); when creatinine clearance is 10-50ml/min, use 30-70% of the normal dose, once every 12-18 hours;
Adverse Reactions
1 Hearing loss, tinnitus or a feeling of fullness in the ears may occur during the treatment, which is due to the impact on the cochlear nerve. A small number of patients, especially those with renal impairment, may develop these symptoms after discontinuation of the drug, so attention should be paid. Dizziness and unsteady gait may occur when the vestibular nerve function is affected, but they are rare. 2 Nephrotoxic reactions such as hematuria, decreased urination frequency or urine volume, loss of appetite, nausea, vomiting, and extreme thirst may occur. 3 Neuromuscular blockade such as dyspnea, drowsiness or weakness may occasionally occur. 4 Other adverse reactions include: headache, rash, drug fever, numbness around the mouth, leukopenia, eosinophilia, local pain after intramuscular injection, etc.
Precautions
Patients with a history of allergy to this product or other aminoglycoside drugs are contraindicated.
Drug Interactions
1. When used in combination with other aminoglycosides or applied topically or systemically one after the other, it may increase ototoxicity, nephrotoxicity and neuromuscular blocking effects. 2. When used in combination with neuromuscular blockers, it may aggravate neuromuscular blocking effects, leading to muscle weakness, respiratory depression, etc. 3. When used in combination with capreomycin, cisplatin, ethacrynic acid, furosemide or vancomycin (or norvancomycin), or applied topically or systemically one after the other, it may increase ototoxicity and nephrotoxicity. 4. When used topically or systemically with cephalothin or cefazolin, it may increase nephrotoxicity. 5. When used in combination with polymyxin injections, or applied topically or systemically one after the other, it may increase nephrotoxicity and neuromuscular blocking effects. 6. Other nephrotoxic and ototoxic drugs should not be used in combination with this product or applied one after the other, so as not to aggravate nephrotoxicity or ototoxicity. 7. When aminoglycosides and beta-lactams (cephalosporins and penicillins) are mixed, they may lead to mutual inactivation. When this product is used in combination with the above-mentioned antibiotics, it must be dripped in separate bottles. It is also not suitable to be infused in the same bottle with other drugs.
