Cytarabine is a purine nucleoside antiviral drug, which is extracted from Streptomyces culture liquid and then synthesized by chemical synthesis. It is a white crystalline powder, very slightly soluble in water, and its monophosphate is easily soluble in water. It inhibited DNA viruses such as herpes simplex virus HSV1 and HSV2, hepatitis B virus, varicella-zoster virus and cytomegalovirus, but not smallpox virus, adenovirus, other DNA or RNA viruses, bacteria and fungi. The exact antiviral mechanism of cytarabine has not been fully elucidated at present, mainly related to the inhibition of viral replication. Drugs and their metabolites block the synthesis of viral DNA by inhibiting the DNA polymerase of the virus, and the drugs themselves only penetrate into the DNA molecules of the virus in a very small amount. In human beings, the antiviral effect of drugs partly depends on the immune function of the host, and drugs have no immunosuppressive effect. 75% ~ 87% of the drugs administered by intravenous drip in vivo are rapidly deaminated to arabinose hypoxanthine by adenosine deoxyenzyme, and the antiviral activity is significantly lower than the original form, and it is quickly distributed in some tissues. When 10mg was administered per kg of body weight, the peak blood concentration of arabinose hypoxanthine was 3 ~ 6 μg/ml, and the peak blood concentration of adenosine arabinose 0.2-0.4 μg/ml. Arabinose hypoxanthine could penetrate the blood-brain barrier, and the concentration of cerebrospinal fluid was about 1/3 of the plasma concentration. 41% to 53% of the daily dose is excreted in the urine in the form of arabinose hypoxanthine, and 1% to 3% is excreted as the prototype drug. Cytarabine is mainly used in the treatment of herpes simplex virus encephalitis, and also used in the treatment of herpes zoster and varicella infection in immunosuppressed patients, but it is not effective against cytomegalovirus. It also has pharmacological activity of inhibiting HBV replication. It is used topically for herpes simplex keratitis and occasionally for vaccinia keratitis.
