Introduction
Sunitinib is also known as Sunitinib Maleate and Sutent. It is a multi-target tyrosine kinase inhibitor that acts on PDGFRα, PDGFRβ, VEGFR1, VEGFR2, VEGFR3, KIT, FLT3, CSFChemicalbook-1R and RET among others, inhibiting the dysregulation of RTKs expression in tumor cells and anti-tumor angiogenesis. It is metabolized by cytochrome P450 enzymes. It is applicable for patients with gastrointestinal stromal tumors who are resistant to imatinib or have progressed after treatment, or for the treatment of renal cell carcinoma.

Indications for use
For gastrointestinal stromal tumors that have failed to respond to imatinib mesylate treatment or are intolerable, as well as for advanced renal cell carcinoma that is inoperable.

In vitro study
Sunitinib can effectively inhibit Kit and FLT-3. Sunitinib is an effective ATP competitive inhibitor for VEGFR2 (Flk1) and PDGFRβ. The Ki values for VEGFR2 and PDGFRβ are 9 nM and 8 nM respectively. Its effects on VEGFR2 and PDGFRβ are much higher than those on FGFR-1, EGFR, Cdk2, Met, IGFR-1, Abl, and src. In NIH-3T3 cells expressing VEGFR2 or PDGFRβ under serum starvation, Sunitinib inhibits VEGF-dependent VEGFR2 phosphorylation and PDGF-dependent PDGFRβ phosphorylation, with IC50 values of 10 nM and 10 nM respectively. For NIH-3T3 cells overexpressing PDGFRβ or PDGFRα, Sunitinib inhibits VEGF-induced proliferation, with IC50 values of 39 nM and 69 nM respectively. Sunitinib inhibits the phosphorylation of wild-type FLT3, FLT3-ITD, and FLT3-Asp835, with IC50 values of 250 nM, 50 nM, and 30 nM respectively. Sunitinib inhibits the proliferation of MV4;11 and OC1-AML5 cells, with IC50 values of 8 nM and 14 nM respectively, and induces apoptosis in a dose-dependent manner.

In vivo studies
The inhibition of VEGFR2 or PDGFR in vivo by substantial and selective manner was consistent with the phosphorylation and signal transduction. Sunitinib (20-80mg/kg/day) exhibited extensive and effective dose-dependent anti-tumor activity in various tumor xenograft models, including HT-29, A431, Colo205, H-460, SF763T, C6, A375, or MDA-MB-435. When administered at a dose of 80mg/kg/day for 21 days, Sunitinib led to complete tumor regression in 6 out of 8 mice, and no tumor recurrence was observed during the 110-day observation period after treatment. The second round of Sunitinib treatment still showed effective anti-tumor activity, but it could not fully recover to the condition of the first round of treatment. Sunitinib treatment resulted in a significant decrease in tumor MVD, reducing by approximately 40% in SF763T glioma. SU11248 treatment completely inhibited the additional tumor growth of PC-3M xenografts expressing luciferase, although the tumor size did not decrease. In the FLT3-ITD bone marrow transplantation model, Sunitinib treatment (20mg/kg/day) significantly inhibited the growth of subcutaneous MV4;11 (FLT3-ITD) xenografts and prolonged survival.
