Medetomidine is a potent and highly selective α2-adrenoceptor (α2-AR) agonist used primarily as a sedative, analgesic, and anxiolytic agent in veterinary medicine, with some applications in human clinical sedation.

Mechanism of Action

Medetomidine exerts its effects by selectively binding to α2-adrenoceptors, which are G protein-coupled receptors with three main subtypes: α2A, α2B, and α2C. These receptors are distributed differently in the central and peripheral nervous systems:
α2A and α2C receptors are predominantly located in the central nervous system (CNS) and mediate sedation, analgesia, and sympatholytic effects by decreasing noradrenergic neuronal firing, particularly in the locus coeruleus.
α2B receptors are mainly found on vascular smooth muscle and mediate vasopressor effects such as initial hypertension.
Activation of these receptors inhibits adenylate cyclase activity, reduces cyclic AMP levels, and decreases neuronal firing, resulting in sedation, analgesia, anxiolysis, and reduced sympathetic outflow. The analgesic effects occur both centrally and peripherally.
Medetomidine is an imidazole derivative and is more potent and selective than older α2-agonists like xylazine, with approximately 200 times greater potency. It is a racemic mixture of two enantiomers: dexmedetomidine, which is responsible for the pharmacological effects, and levomedetomidine, which is largely inactive.

Applications

Veterinary medicine: Used extensively as a sedative and analgesic in dogs, cats, and other animals for clinical examinations, minor surgeries, and anesthesia premedication. It provides reliable sedation and analgesia with muscle relaxation.
Human medicine: Dexmedetomidine, the active enantiomer, is FDA-approved for sedation of mechanically ventilated ICU patients and procedural sedation in non-intubated patients.
Other uses: Medetomidine has been reported as an adulterant in illicit drug supplies, often mixed with opioids like fentanyl.

Routes of Administration and Dosage

Intramuscular (IM) and subcutaneous (SC): Common in veterinary practice for sedation and analgesia.
Intravenous (IV): Used in both veterinary and human clinical settings, often with an initial loading dose followed by continuous infusion.

Safety and Side Effects

Cardiovascular effects: Common side effects include bradycardia, bradyarrhythmias, an initial transient hypertension followed by prolonged hypotension, and reduced cardiac output.
Other side effects: Dry mouth, hypothermia, spontaneous muscle contractions, and respiratory depression.
Toxicity: Overdose can exacerbate cardiovascular depression; the impact of injection speed on toxicity is unclear.
Long-term effects: Limited research exists on chronic use in humans; some concerns exist based on related α2-agonists about possible skin ulcers.

Medetomidine is highly lipophilic, allowing rapid penetration into the CNS.
After administration, peak plasma concentrations occur within minutes to an hour depending on the route.
It has a relatively fast elimination half-life, allowing for quick onset and offset of effects.
Metabolism occurs primarily in the liver, with elimination via urine and feces.
