1. Introduction

Linezolid is an antibiotic used to treat serious bacterial infections. It belongs to the  oxazolidinone  class of antibiotics and is known for its effectiveness against  gram-positive bacteria , including drug-resistant strains.

2. Mechanism of Action

Linezolid works by  inhibiting bacterial protein synthesis .

It binds to the  23S ribosomal RNA of the 50S subunit  of bacterial ribosomes, preventing the formation of a functional  70S initiation complex , which is essential for bacterial replication.

Because of this unique mechanism, cross-resistance with other antibiotic classes is rare.

3. Indications (Uses in Medicine)

Linezolid is primarily used for infections caused by drug-resistant gram-positive bacteria, including:

Pneumonia  (Hospital-acquired & Community-acquired)

Skin & Soft Tissue Infections , including complicated cases

Methicillin-resistant Staphylococcus aureus (MRSA) infections

Vancomycin-resistant Enterococcus (VRE) infections

4. Dosage & Administration

Common standard dosage:

Serotonin Syndrome Risk : Co-administration with  serotonergic drugs  (e.g., fluoxetine, paroxetine) can lead to  excess serotonin levels , causing dangerous symptoms.

Avoid consuming  high-tyramine foods  (e.g., aged cheese, fermented foods) due to the risk of  hypertensive crisis .

7. Precautions & Contraindications

Not recommended for long-term use (>28 days) without close monitoring  due to risk of neuropathy and bone marrow suppression.

Patients with uncontrolled hypertension , pheochromocytoma, or taking MAO inhibitors should use it cautiously.

Resistance to Linezolid is  still relatively low , but improper use can contribute to resistance development.
