Clinical Uses and Applications Calcifediol monohydrate has been used for over 40 years in Spain and has recently become available in several other European countries and Latin America. It is primarily employed in pharmaceutical applications for the treatment of vitamin D deficiency, secondary hyperparathyroidism, and metabolic bone diseases such as osteomalacia and rickets. For supplemental use in the healthy population, calcifediol can be used at doses of up to 10 µg per day for children ≥ 11 years and adults, and up to 5 µg/day in children 3-10 years. In renal failure, the duration of action is extended by 2 to 3 times. The half-life is 10 to 22 days in healthy individuals, but extends to approximately 25 days in patients with stage 3 or 4 chronic kidney disease. Calcifediol is extensively bound to plasma proteins (>98%) and has a mean apparent volume of distribution of 8.8 L in healthy subjects. Elimination occurs primarily through the biliary fecal route. Side Effects and Safety Profile Calcifediol is generally well-tolerated when administered within therapeutic doses. However, excessive exposure may lead to hypercalcemia, which can cause symptoms including nausea, vomiting, weakness, and long-term kidney damage.
