Description

Progesterone exerts significant morphological effects on the oestrogen-stimulated endometrium, being essential for maintaining pregnancy. During the late menstrual phase, it promotes endometrial development and proliferation alongside oestrogen, transitioning it from the proliferative to the secretory phase, thereby creating conditions conducive to fertilised egg implantation and embryonic development. During menstruation, it facilitates complete endometrial shedding. During pregnancy, it inhibits uterine contractions and sensitivity to oxytocin, exerting a tocolytic effect. Concurrently with oestrogen, it promotes further mammary gland development, preparing for lactation. At high doses, it acts via feedback regulation of the hypothalamic-pituitary-ovarian axis to suppress pituitary secretion of luteinising hormone, thereby inhibiting ovulation and providing contraceptive efficacy. Progesterone may be employed for recurrent miscarriage, threatened miscarriage, functional uterine bleeding, menorrhagia, dysmenorrhoea, endometriosis, amenorrhoea, and ureteral calculi. It is also indicated for benign prostatic hyperplasia, sleep apnoea syndrome, and refractory ascites in cirrhosis.
