It is structurally related to the anthracycline antibiotics, such as doxorubicin, but was designed to exhibit comparable efficacy with a potentially improved safety profile, particularly regarding cardiotoxicity. Despite this intention, significant cardiotoxicity remains a serious risk.

  • Acute Leukemia: Specifically, acute myeloid leukemia (AML).

• Multiple Sclerosis (MS): It is approved for the treatment of secondary progressive, progressive relapsing, or worsening relapsing-remitting MS. It works in MS by suppressing the activity of T-cells, B-cells, and macrophages, which are involved in the demyelinating autoimmune process.

Important Safety Considerations:
Mitoxantrone is associated with several serious, dose-dependent side effects:

• Myelosuppression: A significant suppression of bone marrow activity, leading to low blood cell counts (leukopenia, neutropenia, thrombocytopenia).

• Cardiotoxicity: It can cause irreversible congestive heart failure and a reduction in left ventricular ejection fraction (LVEF). Lifetime cumulative dose is strictly monitored.
