Strontium ranelate More Info

Strontium Ranelate is an orally active, dual-acting bone agent used clinically for the treatment of post-menopausal osteoporosis and male osteoporosis. It combines anabolic (bone-forming) and anti-catabolic (anti-resorptive) properties in a single molecule.

Mechanism of Action:

The Sr²⁺ ion is incorporated into hydroxyapatite, increases BMD, and shifts osteoblast/osteoclast balance toward formation; the ranelate anion binds CaSR, down-regulates RANKL, up-regulates OPG, and activates Wnt/β-catenin signaling, yielding net anabolic effect.

Applications

Approved for post-menopausal and male osteoporosis; investigational in glucocorticoid-induced osteoporosis and knee OA.

Routes & Dosage

2 g oral granules once daily ≥ 2 h after food/calcium; Tmax 3 – 5 h, bioavailability ≈ 25 %.

Side Effects:

Common: mild GI upset; less common: QT prolongation, ↑ VTE risk. Contraindicated in current/prior VTE, severe renal failure.

Pharmacokinetics

Sr²⁺ deposits in bone (t½ ≈ 10 y); ranelate excreted unchanged (renal 66 %, fecal 34 %); no hepatic metabolism.
