Pyrimethamine (Daraprim) is a medication used for protozoal infections. It is commonly used as an antimalarial drug (for both treatment and prevention of malaria), and is also used (combined with sulfadiazine) in the treatment of Toxoplasma gondii infections in immunocompromised patients, such as HIV-positive individuals. It is also currently being evaluated in clinical trials as a treatment for ALS. In 2011, researchers have discovered that Pyrimethamine can increase ß-hexosaminidase activity, thus slowing down the progression of Late-Onset Tay-Sachs disease.

Pyrimethamine is a white crystalline powder; odorless and tasteless. It is slightly solubleand almost insoluble in water.

Pyrimethamine can inhibit the dihydrofolate reductase of Plasmodium vivax, thus interfering with the normal folic acid substitution of Plasmodium vivax, which is effective against Plasmodium falciparum and Plasmodium falciparum pre-erythrocytic stage, and is often used as etiological prophylaxis. In addition, it can also inhibit the development of Plasmodium in mosquitoes, so it can interrupt transmission. It is used clinically for malaria prophylaxis and anti-relapse treatment in the resting phase.

Pyrimethamine is an antiparasitic medication belonging to the diaminopyrimidine class of compounds. It is primarily used in the treatment and prevention of malaria caused by Plasmodium falciparum and Plasmodium vivax. Pyrimethamine acts by selectively inhibiting the enzyme dihydrofolate reductase (DHFR) in protozoan parasites, thereby disrupting nucleic acid synthesis and parasite replication. Unlike other dihydrofolate inhibitors, pyrimethamine exhibits a significantly higher affinity for parasitic DHFR compared to the human enzyme, allowing for therapeutic selectivity. The World Health Organization includes pyrimethamine on its List of Essential Medicines. In clinical practice, it is almost always administered in combination with sulfadoxine (as sulfadoxine-pyrimethamine) or with sulfonamide antibiotics to achieve synergistic antimalarial effects.
