    Description

Calcipotriol, a synthetic derivative of calcitriol or Vitamin D, is used in the treatment of psoriasis and marketed under the trade name Dovonex. It has comparable affinity with calcitriol (Vit. D) for the Vitamin D receptor (VDR), while being less than 1% as active as the calcitriol in regulating calcium metabolism. Chronic plaque psoriasis is the chief medical use of calcipotriol. It has also been used successfully in the treatment of alopecia areata.

    Calcipotriol More Info

Other Names: Calcipotriene (US English spelling); brand names include Dovonex, Daivonex, Sorilux (topical foam)Drug Class: Synthetic vitamin D3 analog (vitamin D receptor agonist)

    Core Introduction

Calcipotriol is a synthetic derivative of calcitriol (the active form of vitamin D3), a key pharmaceutical agent primarily used in the topical treatment of psoriasis vulgaris—one of the most common chronic inflammatory skin disorders. It exerts therapeutic effects by regulating keratinocyte proliferation and differentiation, and inhibiting inflammatory cell activity in the skin, thereby reducing the red, scaly, and itchy plaques characteristic of psoriasis.

Unlike systemic vitamin D supplements, calcipotriol is designed for topical cutaneous use (rarely systemic) with minimal systemic absorption, which significantly lowers the risk of hypercalcemia (elevated blood calcium levels)—a major side effect of excessive vitamin D intake. It is one of the first-line topical treatments for mild to moderate plaque psoriasis and is often combined with topical corticosteroids for synergistic effects (e.g., reducing steroid reliance and improving efficacy).

    Mechanism of Action

Modulates keratinocyte function: Binds to vitamin D receptors (VDRs) in epidermal keratinocytes, inhibiting their abnormal overproliferation (a hallmark of psoriasis) and promoting their normal differentiation into mature, healthy skin cells.

Anti-inflammatory activity: Suppresses the activation and migration of inflammatory cells (e.g., T lymphocytes, macrophages) in the skin, and reduces the production of pro-inflammatory cytokines (e.g., TNF-α, IL-23) that drive psoriasis pathogenesis.

Immunoregulation: Regulates the local cutaneous immune response, restoring the balance of the skin’s immune microenvironment.

    Indications

First-line topical treatment for mild to moderate plaque psoriasis vulgaris (adults and pediatric patients ≥6 years old in most regions).

May be used as an adjunct therapy for severe psoriasis in combination with phototherapy (e.g., UVB) or systemic medications (e.g., methotrexate) to enhance local symptom control.

Not indicated for other psoriasis subtypes (e.g., pustular, erythrodermic psoriasis) or non-psoriatic skin conditions without specific clinical evidence.

    Formulations & Administration

Calcipotriol is available in topical formulations only (no oral/injectable forms) for cutaneous application, with common preparations:

Ointment: 0.005% w/w (standard concentration); for thick, chronic psoriatic plaques (e.g., elbows, knees, scalp).

Cream: 0.005% w/w; for milder, more sensitive skin areas (e.g., face, neck, flexural folds).

Scalp solution/lotion: 0.005% w/w; for scalp psoriasis (easy to apply to hair-bearing areas).

Topical foam: For patients who prefer a non-greasy formulation, suitable for large skin areas.

Administration guidelines: Apply a thin layer to affected skin areas 1–2 times daily, gently rub until absorbed. Avoid contact with eyes, mouth, and mucous membranes; wash hands after application (unless treating hand psoriasis). The maximum weekly dose is usually limited (e.g., 100g of ointment/cream) to prevent systemic absorption and hypercalcemia.

    Adverse Effects

Adverse effects are mostly local, mild, and transient, with systemic side effects rare when used as directed:

Local skin reactions (most common): Burning, stinging, itching, redness, dryness, or peeling at the application site—usually occurs in the first 1–2 weeks of treatment and resolves with continued use.

Rare local effects: Irritant contact dermatitis, folliculitis, or worsening of psoriasis (in rare cases).

Systemic effects (very rare): Hypercalcemia, hypercalciuria (increased calcium in urine) — typically only with excessive, prolonged use or application to large, denuded skin areas. Symptoms include fatigue, nausea, vomiting, increased thirst/urination.

Contraindications: Hypersensitivity to calcipotriol or any excipients in the formulation; severe renal/hepatic impairment (due to potential calcium metabolism disruption); hypercalcemia/hypercalciuria.

    Drug Interactions

Topical corticosteroids: Combined use is common and recommended—corticosteroids reduce local irritation from calcipotriol, while calcipotriol enhances the anti-psoriatic effect and reduces steroid-induced skin atrophy (long-term use risk).

Other vitamin D analogs: Concurrent systemic use (e.g., calcitriol supplements) increases the risk of hypercalcemia—avoid combination unless closely monitored by a clinician.

Phototherapy (UVB): Synergistic effect when combined, but may increase skin photosensitivity—adjust treatment frequency/dosage under medical supervision.

    Clinical Notes

Pediatric use: Approved for children ≥6 years old (plaque psoriasis) with strict dose limits—avoid use in infants/young children due to immature calcium metabolism.

Pregnancy/lactation: Classified as a pregnancy category B (US) or C (EU) — use only if the potential benefit outweighs the risk, under clinician supervision. Minimal systemic absorption means low risk to the fetus/infant, but no long-term safety data are available.

Scalp use: The scalp solution/lotion is formulated to penetrate hair follicles; avoid contact with the eyes (may cause conjunctivitis).

Monitoring: For long-term or high-dose use, periodic monitoring of serum calcium and urine calcium levels is recommended to detect early signs of hypercalcemia.

    Pharmacokinetics

Absorption: Minimal percutaneous absorption (≤5%) when applied to intact skin—absorption increases slightly in inflamed/psoriatic skin but remains low. No significant systemic accumulation with regular use.

Metabolism: Metabolized locally in the skin to inactive metabolites; small amounts of absorbed drug are metabolized in the liver (similar to calcitriol).

Elimination: Excreted primarily in the feces, with a small portion in urine; no significant accumulation in renal/hepatic tissues in healthy individuals.
